髋关节置换术中应用全身麻醉与硬腰联合麻醉的术中及术后效果的比较

Intraoperative and postoperative effects comparison between general anesthesia and hard waist combined anesthesia in the treatment of hip arthroplasty

目的探究髋关节置换术中应用全身麻醉与硬腰联合麻醉的术中及术后效果。方法选择在我院开展髋关节置换术的患者84例,按照就诊先后顺序将患者分成联合组及全麻组,每组42例,分别施行硬腰联合麻醉术及全身麻醉术,对比两组的麻醉效果。结果两组患者的术中出血量、输血量、输液量及手术时间比较,差异均不显著(P>0.05);联合组恢复规律呼吸时间、术后睁眼时间、摘除喉罩时间及恢复定向力时间明显短于全麻组,躁动发生率低于全麻组,摘除喉罩即刻及5 min后的血压、心率、血氧饱和度均优于全麻组,差异有统计学意义(P<0.05),摘除喉罩后10 min两组血压、心率、血氧饱和度比较,差异不显著(P>0.05)。结论相对于仅用全身麻醉而言,硬腰联合麻醉在促进髋关节置换术后血流动力学指标恢复方面更具优势,能够促使患者术后苏醒期情绪平稳不躁动,缩短苏醒时间,在具备手术指征的前提下可作为髋关节置换术的首选麻醉术式。

Objective To explore the intraoperative and postoperative effects of general anesthesia and hard waist combined anesthesia in the treatment of hip arthroplasty.Methods Eighty-four cases of patients undergoing hip arthroplasty in our hospital were enrolled,and divided into combination group and general anesthesia group according to admission order,with 42 cases in each group.The combination group was treated with hard waist combined anesthesia,and the general anesthesia group accepted general anesthesia,and the anesthetic effects between the two groups were compared.Results There were no significant differences in intraoperative blood loss,blood transfusion,infusion volume and operation time between the two groups(P>0.05).The resuscitation regular respiration time,postoperative open eye time,laryngeal mask removal time and directional force recovery time in the combination group were significantly shorter than those in the general anesthesia group(P<0.05);the incidence of agitation was lower than that in the general anesthesia group(P<0.05);the blood pressure,heart rate and oxygen saturation of the laryngeal mask removed immediately and at 5 min after laryngeal mask removal were better than those in the general anesthesia group(P<0.05),there were no significant differences in the above indexes between the two groups at 10 min after laryngeal mask removal(P>0.05).Conclusion Hard waist combined anesthesia has more advantages in the treatment of hip arthroplasty than general anesthesia,which can promote recovery of hemodynamics,promote patients to retain emotional instability restlessness after waking up,shorten the recovery time and can be used as the preferred anesthesia surgery under the premise of hip surgery.
