巨细胞病毒活动性感染与婴儿肝功能损害的相关性研究

Relationship study between cytomegalovirus active infection and infant liver function impairment

目的研究巨细胞病毒活动性感染与婴儿肝功能损害的相关性。方法选取我院收治的224例婴儿肝炎综合征患儿为肝炎综合征组,按照年龄又分为<3个月组与≥3个月组,各112例。以同期来我院体检的正常患儿224例为对照组。采用ELISA定量法以及荧光定量PCR法对患儿进行巨细胞病毒Ig M抗体及DNA实施定量检测。并对巨细胞病毒感染阳性患儿进行肝功能(TBIL、ALT、AST、GGT)检测。结果肝炎综合征组患儿巨细胞病毒DNA和Ig M抗体检测阳性率均显著高于对照组,且DNA阳性率高于Ig M抗体检测阳性率(P<0.05);肝炎综合征组内,<3个月组患儿巨细胞病毒Ig M抗体及DNA阳性率均明显高于≥3个月组(P<0.05)。肝炎综合征组患儿各肝功能各指标水平均高于对照组(P<0.05),肝炎综合征组内,<3个月组患儿GGT及TBIL水平和≥3个月组比较,具有显著差异(P<0.05)。结论肝炎综合征与巨细胞病毒存在高度相关性,<3个月的肝炎综合征患儿巨细胞病毒检出率较≥3个月患儿更高。巨细胞病毒早期检出对婴儿感染早期诊断和治疗具有十分重要的意义。

Objective To study the relationship between cytomegalovirus active infection and infant liver function impairment.Methods A total of 224 children with suspected cytomegalovirus(CMV)in our hospital were enrolled as the hepatitis syndrome group.Then,those children were divided into<3 months group and≥3 months group according to the age,with 112 cases in each group.And 224 cases of normal children who took physical examination in our hospital in the same time period were enrolled as the control group.The ELISA quantitative and fluorescent quantitative PCR were used to detect the CMV IgM antibody and CMV DNA in the children.Liver function(TBIL,ALT,AST and GGT)were measured in patients with positive cytomegalovirus infection.Results The positive rate of CMV DNA and CMV IgM antibody in hepatitis syndrome group were significantly higher than those in the control group,and the positive rate of CMV DNA was higher than that of CMV IgM antibody(P<0.05).In the hepatitis syndrome group,the positive rates of CMV IgM antibody and CMV DNA of CMV in the<3 months group were significantly higher than those of the≥3 months group(P<0.05).The levels of liver function indexes in children in the hepatitis syndrome group were higher than those in the control group(P<0.05).In the hepatitis syndrome group,there were significant differences in the leves of GGT and TBIL between the≥3 months group and the<3 months group(P<0.05).Conclusion There is a high correlation between CMV and cytomegalovirus in children with hepatitis syndrome.The detection rate of CMV patients in<3 months old is higher than that in≥3 months old.Early detection of CMV is very important for the early treatment and diagnosis of infant infection.
