快速康复外科理念指导围手术期护理对四肢骨折患者预后的影响

Effect of the rapid rehabilitation surgery concept in perioperative nursing on the prognosis of patients with limbs fracture

目的探讨以快速康复外科理念指导围手术期护理对四肢骨折患者预后的影响。方法回顾分析2015年1月至2016年12月在民勤县人民医院手术治疗的361例四肢骨折患者的临床护理资料,其中204例患者应用了快速康复外科理念指导围手术期护理,为观察组;另157例患者给予常规护理,为对照组。比较两组患者术后并发症、不良反应、疼痛评分、住院时间,随访比较两组骨折愈合时间及生活质量。结果观察组术后并发症及不良反应发生率均低于对照组(P<0.05),术后住院时间及骨折愈合时间也短于对照组(P<0.05)。两组术后疼痛评分均逐渐下降,但观察组低于对照组(P<0.05)。术后1、3个月观察组的生理职能、社会功能、情感职能、精神健康评分均高于对照组(P<0.05)。结论应用围手术期快速康复外科理念指导围手术期护理可减少四肢骨折手术患者术后并发症发生率,缩短住院时间及骨折愈合时间,改善患者术后生活质量。

Objective To investigate the effect on the prognosis of patients with limbs fracture guided by the concept of rapid rehabilitation surgery on perioperative nursing.Method From January 2015 to December 2016,361 patients with limbs fracture were treated in Minqin People’s hospital.According to the nursing methods,all of the patients who received surgical treatment were divided into control group and observation group.The patients in control group(n=157)received routine nursing.Meanwhile,204 patients received perioperative nursing guided by the concept of rapid rehabilitation were selected into observation group.The postoperative complications,adverse reactions,postoperative hospitalization time and VAS scores between two groups were compared as well as the postoperative quality of life and fracture healing time.Results The incident rates of complications and adverse reactions of the patients in observation group were significantly lower than those in control group(P<0.05).Postoperative hospitalization time and fracture healing time in observation group were significantly shorter than those in control group(P<0.05).The two groups of postoperative VAS score decreased,and VAS scores of the patients in observation group was significantly lower than that in control group(P<0.05).RP,SF,RE,MH in observation group were significantly higher than those in control group after 1 and 3 months respectively(P<0.05).Conclusions Guided by the concept of rapid rehabilitation surgery in perioperative nursing,the incidence rates of postoperative complications and adverse reactions could be reduced.And the time of hospitalization and fracture healing could be shortened with the postoperative quality of life being improved.
