丁苯酞联合盐酸多奈哌齐治疗脑梗死后血管性痴呆的meta分析

Butylphthalide Combined with Donepezil for Vascular Dementia after Cerebral Infarction:a Meta-Analysis

目的观察丁苯酞联合盐酸多奈哌齐治疗脑梗死后血管性痴呆的有效性与安全性。方法计算机检索Pub Med、EMBase、The Cochrane Library、Wed of Science CBM、CNKI、VIP和万方数据库,选择丁苯酞联合盐酸多奈哌齐治疗脑梗死后血管性痴呆的随机对照试验(randomized controlled trails,RCT)。两组均予盐酸多奈哌齐治疗,在此基础上,观察组加用丁苯肽。由两位研究人员进行文献筛选、数据提取和质量评价,观察简明精神状态检查(MMSE)量表评分、神经心理学测验常用日常生活能力(ADL)量表评分及总有效率。结果本研究共纳入8篇RCT(714例),meta分析显示,与对照组比较,观察组总有效率、MMSE量表评分均升高,ADL量表评分降低,差异均有统计学意义(P<0.05)。结论丁苯肽联合盐酸多奈哌齐可提高血管性痴呆患者的神经功能缺损及日常生活能力,且安全性好。

Objective To evaluate the efficacy and safety of butyphthalide combined with Donepezil in the treatment of vascular dementia after cerebral infarction.Methods PubMed,EMBase,Cochrane Library,Wed of Science CBM,CNKI,VIP and Wanfang Data were searched to collect randomized controlled trials(RCTs)about Butylphthalide combined with Donepezil in the treatment of vascular dementia.Both groups were given Donepezil,and on this basis,the observation group was supplemented with Butylphthalide.Literature screening,data collection and quality evaluation were conducted by two researchers appointed.MMSE scale score,ADL score and total effective rate of neuropsychological tests were documented.According to the evaluation system of Cochrane Handbook,the RCTs included were analyzed using RevMan 5.3 software.Results Eight RCTs were included in the study.Meta-analysis indicated a higher total effective rate and MMSE score but lower ADL scores in experimental group than in control group.A significant difference was found in both groups(P<0.05).Conclusion Butylphthalide combined with Donepezil can improve the neurological deficit and daily living ability of patients with vascular dementia,which guarantees good safety.
