中量幕上高血压脑出血CT定位血肿清除液化引流术与神经外科保守治疗的临床对比

Clinical study on CT-guided hematoma clearance liquefaction drainage and neurosurgical conservative therapy in treatment of medium supratentorial hypertensive intracerebral hemorrhage

目的对比中量幕上高血压脑出血CT定位血肿清除液化引流术与神经外科保守治疗的临床效果。方法回顾性分析我院2015年10月—2017年10月收治的60例中量幕上高血压脑出血患者的临床资料,根据治疗方法不同将其分为2组。观察组30例采用CT定位血肿清除液化引流术治疗,对照组30例采用神经外科保守治疗,比较2组临床疗效、神经功能缺损及日常生活活动能力评分及并发症发生情况。结果观察组总有效率、Barthel指数评分高于对照组,NIHSS评分低于对照组,差异均有统计学意义(P<0.05);对照组并发症发生率16.67(5/30),高于观察组的10.00%(3/30),但差异无统计学意义(P>0.05)。结论相较于神经外科保守治疗,CT定位血肿清除液化引流术可提高中量幕上高血压脑出血患者的临床疗效,降低神经功能损伤,且并发症少,利于改善预后。

Objective To investigate the clinical effects of CT-guided hematoma clearance liquefaction drainage and neurosurgical conservative therapy in treatment of medium supratentorial hypertensive intracerebral hemorrhage.Methods The clinical data of 60 cases of medium supratentorial hypertensive intracerebral hemorrhage from October 2015 to October 2017 were retrospectively analyzed,and all the patients were divided into two groups by treatment methods,30 cases in each group.Observation group was given CT-guided hematoma clearance liquefaction drainage,while control group was treated with neurosurgical conservative therapy.The clinical efficacy,neurological deficit,score of Barthel index and,complications were compared between the two groups.Results After treatment,the total effective rate and score of Barthel index in observation group were higher than those in control group,while the NIHSS score was lower than control group(P<0.05);The incidence of complications in control group[16.67%(5/30)]was higher than observation group[10.00%(3/30)],but there was no statistical difference(P>0.05).Conclusion Compared with neurosurgical conservative therapy,CT-guided hematoma clearance liquefaction drainage in treatment of medium supratentorial hypertensive intracerebral hemorrhage can increase clinical efficacy,decrease neurological deficit,have less complications and facilitate prognosis.
