某医院624例2型糖尿病患者视网膜病变患病率及其危险因素分析

Prevalence and Risk Factors of Retinopathy in 624 Type 2 Diabetic Patients in a Hospital

目的探讨某医院624例2型糖尿病(T2DM)患者视网膜病变(DR)患病率及影响其发生的相关危险因素。方法选取2014年1月至2016年12月于某院门诊及住院部就诊的624例T2DM患者作为研究对象,采用回顾性分析法分析所有患者的临床资料,根据资料结果记录所有患者的一般情况、相关实验室检查、系统眼科检查结果以及DR发生情况,并比较不同DR患者间上述资料的差异性,研究影响T2DM患者发生DR的相关危险因素。结果 (1)本组624例T2DM患者中,检出DR患者168例,占26.92%;(2)合并DR与未合并DR的T2DM患者在年龄、收缩压、舒张压、病程、LDL-C及尿微量白蛋白水平上比较,差异均具有统计学意义(P<0.05);(3)经非条件多因素Logistic回归模型分析得出,年龄过大、血压过高、病程较长、LDL-C和尿微量白蛋白水平过高是导致T2DM患者并发DR的独立危险因素(P<0.05)。结论 T2DM患者DR发病率较高,且年龄过大、血压过高、病程较长、LDL-C和尿微量白蛋白水平过高是导致T2DM患者并发DR的独立危险因素。

Objective To investigate the prevalence of retinopathy(DR)in 624 patients with type 2 diabetes mellitus(T2DM)in a hospital and its related risk factors.Methods A total of 624 T2DM patients treated at a hospital between January 2014 and December 2016 were selected as subjects.The clinical data on each of these patients was analyzed retrospectively.According to the results of the data,the general information of patients,relevant laboratory tests,results of systematic eye examinations,and occurrence of DR were recorded.The difference in these data was compared between different DR patients.The related risk factors for DR in patients with T2DM were studied.Results(1)Among the 624 cases of T2DM patients,168 cases of DR were detected,accounting for 26.92%.(2)There was significant difference in age,systolic blood pressure,diastolic blood pressure,duration of disease,LDL-C and urine microalbuminuria between T2DM patients with and without DR(P<0.05).(3)Logistic regression analysis using non-conditional multivariate analysis showed that old age,high blood pressure,longer duration,high levels of LDL-C and high levels of urine microalbumin were independent risk factors(P<0.05)for concurrent DR in T2DM.Conclusion The incidence of DR in T2DM patients is high.Old age,high blood pressure,a long course of disease,a high level of LDL-C and high level of urine microalbumin are independent risk factors for concurrent DR in T2DM.
