经尿道1470 nm半导体激光汽化术联合自拟前列腺汤对良性前列腺增生患者尿道功能、性功能及生活质量的影响

Effect of transurethral 1470 nm semiconductor laser vaporization and self-designed prostatic soup on urethral function,sexual function and quality of life in patients with benign prostatic hyperplasia

目的观察经尿道1 470 nm半导体激光汽化术联合自拟前列腺汤治疗对良性前列腺增生(BPH)患者尿道功能、性功能及生活质量的影响。方法将104例BPH患者随机分为观察组及对照组各52例,对照组给予经尿道1 470 nm半导体激光汽化术治疗,观察组在对照组的基础上于术后当天给予自拟前列腺汤治疗,治疗至术后8周,观察2组尿道功能、性功能及生活质量改善情况。结果 2组治疗后前列腺体积、国际前列腺症状评分(IPSS)、残余尿量(RUV)、最大尿流率(Qmax)、最大尿道关闭压(MUCP)、膀胱顺应性(BC)均显著改善(P均<0.05),且观察组的改善情况优于对照组(P均<0.05);2组治疗后的α-葡萄糖苷酶(α-Glu)、精浆酸性磷酸酶(ACP)、精浆果糖(Fru)水平和国际勃起功能指数-5(IIEF-5)评分均显著提高(P均<0.05),勃起功能障碍、逆行射精发生率显著降低(P均<0.05),且观察组的改善情况均显著优于对照组(P均<0.05);2组治疗后生活质量综合评定问卷(GQOLI-74)中的物质生活、社会功能、心理功能、躯体功能、总体生活满意度评分均显著升高(P均<0.05),观察组上述维度评分均显著高于对照组(P均<0.05)。观察组术后留置导管时间、术后卧床时间、术后住院时间均显著短于对照组(P均<0.05),并发症发生率显著低于对照组(P<0.05)。结论在经尿道1 470 nm半导体激光汽化术治疗BPH基础上,采用自拟前列腺汤辅助治疗能够显著改善患者尿道功能、尿流动力学、性功能及生活质量,缩短康复时间,减少术后并发症。

Objective:It is to observe the effect of transurethral 1 470 nm semiconductor laser vaporization combined with self-designed prostate soup on urethral function,sexual function and quality of life in patients with benign prostatic hyperplasia(BPH).Methods 0x09 104 patients with BPH were randomly divided into observation group(52 cases)and control group(52 cases).The patients were treated with transurethral 1 470nm laser vaporization in the control group,and with self-designed prostatic soup on the basis of the control group after operative in the observation group,the treatment continued for 8 weeks.the improvement of the two groups of urethral functions,sexual function and quality of life were observed.Results 0x09 After treatment,the prostate volume,International Prostate Symptom Score(IPSS),residual urine volume(RUV),maximum urine flow rate(Qmax),maximum urethral closure pressure(MUCP)and bladder compliance(BC)in the two groups were improved significantly(all P<0.05),and the improvement of the observation group was better significantly than that of the control group(P<0.05).After treatment,the levels ofα-glucosidase(α-Glu),seminal acid phosphatase(ACP),seminal plasma fructose(Fru),and the score of international erectile function index-5(IIEF-5)in the two groups were improved significantly(all P<0.05),and the incidence of erectile dysfunction and retrograde ejaculatory ejaculation were decreased significantly(P<0.05),and the improvement of the observation group was better than that of the control group(P<0.05).The quality of life,social function,psychological function,physical function and overall life satisfaction score of GQOLI-74 in the two groups were increased significantly after treatment(all P<0.05),and the scores of the above dimensions in the observation group were higher significantly than those in the control group(all P<0.05).The time of indwelling catheter,postoperative bed rest and postoperative hospital stay in the observation group were shorter significantly than those in the control group(all P<0.05),and the incidence of complications was lower significantly than that in the control group(P<0.05).Conclusion 0x09 On the basis of transurethral 1 470 nm laser vaporization for the treatment of BPH,the adjuvant therapy of prostate soup can significantly improve the urethral function,urinary flow dynamics,sexual function an
