肺康复训练辅助支气管舒张药物治疗中重度慢性阻塞性肺疾病的疗效

Pulmonary Rehabilitation Training Combined with Bronchodilator for Moderate to Severe Chronic Obstructive Pulmonary Disease

目的探讨肺康复训练辅助支气管舒张药物对中重度慢性阻塞性肺疾病(COPD)的治疗效果。方法将60例中重度COPD患者按随机数字表法分为观察组和对照组,每组30例。对照组按常规给予支气管舒张药物治疗,观察组在对照组治疗基础上,给予肺康复训练治疗。比较2组治疗前及治疗6个月后的肺功能指标[用力肺活量(FVC)、第1秒用力呼气容积(FEV1)占预计值百分比(FEV1%)]、血气分析指标[动脉血二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)]、6min步行距离(6MWD)、生活质量(SGRQ)评分、呼吸困难指数(mMRC)。结果2组治疗后FVC、FEV1%、PaCO2、PaO2、6MWD、SGRQ评分、mMRC值均较治疗前显著改善,且观察组改善程度明显优于对照组,差异均有统计学意义(P<0.05)。结论采用肺康复训练辅助支气管舒张药物治疗中重度COPD效果显著,可有效改善肺功能和运动耐力,提高患者生活质量。

Objective To investigate the effect of pulmonary rehabilitation training combined with bronchodilator on moderate to severe chronic obstructive pulmonary disease(COPD).Methods Sixty patients with moderate to severe COPD were randomly treated with bronchodilator alone(control group,n=30)or in combination with pulmonary rehabilitation training(observation group,n=30).The forced vital capacity(FVC),percentage of predicted forced expiratory volume in one second(FEV1%),PaCO 2,PaO 2,6 minute walking distance(6MWD),quality of life(SGRQ)score and degree of dyspnea(mMRC)were observed before and 6 months after treatment.Results The FVC,FEV1%,PaCO 2,PaO 2,6MWD,SGRQ score and mMRC value were improved after treatment in both groups(P<0.05),and the improvement in observation group was more obvious than that in control group(P<0.05).Conclusion Pulmonary rehabilitation training combined with bronchodilator can effectively improve lung function,exercise endurance and quality of life in patients with moderate to severe COPD.
