腹腔镜子宫切除术对子宫肌瘤患者的临床疗效及卵巢功能的影响

Effect of Laparoscopic Hysterectomy on Ovarian Function and Clinical Effect in Patients with Uterine Leiomyoma

目的:探讨子宫切除术对子宫肌瘤患者的临床疗效及卵巢功能的影响。方法:71例子宫肌瘤患者(选自2015年11月至2016年9月)被随机分为观察组和对照组,均行子宫切除术,其中34例观察组患者采取腹腔镜辅助,37例对照组患者采取直接开腹。比较两组的临床疗效、并发症情况及术后第6个月性激素水平。结果:观察组手术时间(144.62±13.17)min长于对照组,术中出血量(85.18±27.32)m L、排气时间(20.17±2.87)h、下床活动时间(21.15±1.63)h、住院天数(8.13±3.05)d少于对照组,且差异具有统计学意义(P<0.05);观察组并发症发生率11.8%低于对照组16.2%,差异无统计学意义(P>0.05);在第6个月,两组雌二醇水平较治疗前均显著降低,观察组降低幅度小于对照组(P<0.05),两组卵泡刺激素、促黄体生成素较治疗前有所升高,观察组升高幅度小于对照组较低(P<0.05)。结论:腹腔镜子宫切除术临床疗效明显、并发症发生率低、对卵巢功能影响小,值得临床进一步推广应用。

Objective:To explore the clinical effects of laparoscopic hysterectomy surgery on post-operative ovarian function.Methods:A total of 71 cases with uterine leiomyoma were chosen from November 2013 to September 2016,recceived either laparoscopic hysterectomy(observation group,n=34)or open hysterectomy(control group,n=37).The clinical effects,complication rate between two groups,and the reproductive hormone level were compared at 6 months after operation.Results:There were significant differences between observation group and control group in operative time,blood loss,postoperative exhaust,postoperative ambulation time and hospital stay(P<0.05).The incidence of complications was lower and postoperative life quality was better in observation group(P>0.05).Estradiol levels were lower than those before treatment in two groups,follicle stimulating hormone and luteinizing hormone levels were higher than those before treatment in two groups,The change range of the observation group was less than that of the control group.Conclusion:Laparoscopic hysterectomy has obvious clinical effect,Low complication rate and little influence on ovarian function,which is worthy of clinical promotion.
