摘要
目的:了解室性早搏对中心动脉压的影响。方法:根据联律间期将行冠状动脉造影术时出现室性早搏的患者分成两组。短联律间期室性早搏(SCIVPB)组(n=32),即R'R/RR<0.6(R'R为窦性心律QRS与室性早搏的间距,RR为窦性心律RR间距);长联律间期室性早搏(LCIVPB)组(n=35),即R'R/RR≥0.6。比较两组室性早搏时中心动脉压的变化及室性早搏前窦性心律时中心动脉舒张压(CDP)的变化。结果:两组室性早搏时中心动脉收缩压(CSP)和CDP均较窦性心律时显著下降(P均<0.05)。与LCIVPB组相比,SCIVPB组CSP和CDP的下降率更大(P均<0.05)。与窦性心律时相比,两组室性早搏前窦性心律时的CDP增高,但SCIVPB组较LCIVPB组增高率更显著(P均<0.05)。结论:SCIVPB和LCIVPB时中心动脉压下降和外周阻力增高,且SCIVPB时变化更显著。
Objective:To study the impact of ventricular premature beat(VPB)on central aortic pressure(CAP).Methods:According to the coupling interval,the patients with VPB during coronary angiography were divided into two groups:short coupling interval ventricular premature beat(SCIVPB)group,as R'R/RR〈0.6(R'R is the interval between QRS of sinus rhythm and ventricular premature,RR is the interval between sinus rhythm),for 32 patients,and long coupling interval ventricular premature beat(LCIVPB)group,as R'R/RR≥O.6,for 35 patients.The change of CAP in VPB and the central aortic diastolic pressures(CDP)of sinus rhythm before the VPB in two groups were compared.Results:The central aortic systolic pressures(CSP)and CDP in VPB were significantly lower than those in sinus rhythm in two groups(all P<0.05).The decreased percentages of the CSP and CDP in SCIVPB group was significantly higher than those in LCIVPB group(both P<0.05).The CDP of sinus rhythm before the VPB was significantly higher than that of sinus rhythm in two groups.The increased percentage of the CDP in SCIVPB group was significantly higher than that in LCIVPB group(all P〈0.05).Conclusions:The CAP decreased and the peripheral resistance increased in VPB,and the change of SCIVPB was more obvious than that of LCIVPB.
