不同助产方式配合无保护会阴接生的临床分析

Clinical analysis on different midwifery methods combined with unprotected perineum delivery

目的:探讨不同助产方式对无保护会阴接生的效果。方法:收治自然分娩初产妇628例,随机分为A、B、C、D 4组,各157例。A组进行单纯无保护会阴接生,B组进行双侧会阴阻滞麻醉配合无保护会阴接生,C组进行导乐助娩配合无保护会阴接生,D组进行双侧会阴阻滞麻醉和导乐助娩共同配合无保护会阴接生。结果:D组接生成功率、分娩疼痛程度以及会阴完整率显著优于其余3组(P<0.05)。结论:使用双侧会阴阻滞麻醉和导乐助娩共同配合无保护会阴接生分娩临床效果较好,可显著提升分娩成功率,降低产后疼痛感,并可提高会阴完整率。

Objective:To investigate the effect of different ways of midwifery on the unprotected perineum delivery.Methods:628 cases of natural childbirth primiparas were randomly divided into four groups:A,B,C,and D,with 157 cases each group.The A group were treated with unprotected perineum delivery;the B group were given bilateral perineum block anesthesia combined with no protection of perineum delivery;the C group with Doula help delivery of unprotected perineum delivery;the D group of bilateral perineum block anesthesia and Doula help delivery together with no protection of perineum.Results:The success rate of delivery,the degree of pain and the percentage of perineum in the D group were significantly better than those in the other 3 groups(P<0.05).Conclusion:Bilateral perineum block anesthesia and Doula help delivery together with unprotected perineum delivery delivery clinical effect is good,can significantly improve the delivery success rate,reduce postpartum pain,and can improve the integrity rate.
