活血解毒汤联合穴位按摩用于化脓性阑尾炎患者术后胃肠功能恢复的临床效果以及对血清PCT和CRP水平的影响

Clinical efficacy of Huoxue Jiedu Decoction combined with acupoint massage on the postoperative gastrointestinal function recovery in patients with suppurative appendicitis and the effects on serum PCT and CRP levels

目的探讨活血解毒汤联合穴位按摩在化脓性阑尾炎患者术后胃肠功能恢复的临床效果及对血清PCT、CRP水平的影响。方法选择2015年6月—2017年8月自愿到科室接受治疗的化脓性阑尾炎患者104例,随机数字表法分为对照组(n=52)和观察组(n=52)。对照组采用腹腔镜下阑尾切除术治疗,观察组在对照组基础上联合活血解毒汤、穴位按摩治疗,记录2组术后肠鸣音、首次排便及首次排气时间;采用固相免疫色谱法检测2组血清PCT水平;采用免疫比浊法测定2组CRP水平,比较2组临床效果及对术后胃肠功能恢复、血清PCT和CRP水平的影响。结果观察组与对照组均行腹腔镜阑尾切除术,2组手术时间、住院时间及术中出血量水平无统计学意义(P>0.05);观察组术后肠蠕动恢复、肛门排气及首次排便时间,均短于对照组(P<0.05);2组手术前血清PCT、CRP水平比较无统计学意义(P>0.05);观察组术后PCT、CRP水平,均低于对照组(P<0.05);观察组术后并发症发生率为7.69%和对照组9.62%比较无统计学意义(P>0.05)。结论将活血解毒汤联合穴位按摩用于化脓性阑尾炎患者术后效果理想,有助于促进胃肠功能恢复,降低血清PCT、CRP水平,值得推广应用。

Objective To investigate the clinical efficacy of Huoxue Jiedu Decoction combined with acupoint massage on the recovery of the gastrointestinal function of patients with suppurative appendicitis and the effects on serum PCT and CRP levels.Methods A total of 104 patients with purulent appendicitis who underwent laparoscopic appendectomy during June 2015 and Aug.2017 were randomly divided into the control group(n=52)and observation group(n=52).The observation group received Huoxue Jiedu Decoction and acupoint massage.The postoperative bowel sounds,the first defecation time and exhaust time of the two groups were recorded.The serum PCT was detected with solid phase immune chromatography.The serum CRP was detected with immunoturbidimetry.Results There were no differences in the operation time,hospitalization time,blood loss and incidence of postoperative complications between the two groups(P>0.05).The observation group needed shorter time for postoperative bowel recovery,first exhaust and defecation than the control group(P<0.05).Before treatment,there were no statistical differences in serum PCT and CRP levels between the two groups(P>0.05).After treatment,the PCT and CRP levels in the observation group were lower than those in the control group(P<0.05).Conclusion Huoxue Jiedu Decoction combined with acupoint massage is effective for the recovery of patients after laparoscopic appendectomy,which can promote the recovery of gastrointestinal function and reduce the serum PCT and CRP levels,so it is worthy of popularization and application.
