复方恒清汤联合马来酸左旋氨氯地平治疗高血压性眩晕的临床疗效

Effects of Compund Hengqing Decoction and Levoamlodipine Maleate Tablets in the Treatment of Hypertensive Vertigo

目的观察复方恒清汤联合马来酸左旋氨氯地平治疗高血压性眩晕的临床疗效。方法将190例高血压性眩晕病人随机分为对照组与治疗组。对照组口服马来酸左旋氨氯地平治疗,治疗组在口服左旋氨氯地平治疗基础上加服复方恒清汤。分析并比较治疗前后血压变化、眩晕评定量表(DARS)评分、症状积分、空腹血糖(FBG)、血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)及安全性指标(血尿便三大常规、肝肾功能及心电图)。结果治疗组总有效率为84.78%,对照组总有效率为68.89%,治疗组疗效优于对照组(P=0.000)。治疗组治疗后收缩压低于对照组(P=0.043);治疗组治疗后舒张压水平低于对照组,但两组比较差异无统计学意义(P=0.067);治疗组DARS积分下降更明显,差异有统计学意义(t=2.358,P=0.017);治疗组FBG、TC、TG、LDL-C均显著下降(P<0.05或P<0.01);治疗组全血低切黏度、全血高切黏度、血浆黏度、全血还原黏度、纤维蛋白原均显著下降(P<0.05或P<0.01);与治疗前比较,治疗组各项症状均明显改善(P<0.05或P<0.01);对照组除乏力有所改善(P<0.05),其余症状差异均无统计学意义(P>0.05)。结论中药复方恒清汤可降低高血压性眩晕病人的血压(以收缩压为明显),改善眩晕症状和糖脂代谢及血液流变学,安全性较高,临床疗效显著。

Objective To explore the effects of compound Hengqing decoction and levoamlodipine maleate tablets s in the treatment of hypertensive vertigo.Methods One hundred and ninety patients with hypertensive vertigo were randomly divided into control group and treatment group.The control group was treated with levoamlodipine maleate tablets,while the treatment group was treated with compound Hengqing decoction and levoamlodipine maleate tablets.Blood pressure before and after the treatment,dizziness assess-ment rating scale(DARS)score,symptom integral,fasting blood glucose(FBG),serum total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),and safety indices(blood routine,urine routine,stool routine,liver function,kidney function,and electrocardiogram).Results The total clinical effective rate was 84.78%in treatment group,which was significantly higher than that in control group(68.89%,P=0.000).After treatment,compared with control group,systolic blood pressure of treatment group decreased more significantly(P=0.043),diastolic blood pressure of the treatment group also decreased significantly,but there was no statistical significance between the two group(P=0.067).DARS integral of treatment group decreased more significantly(t=2.358,P=0.017).FBG,TC,TG,and LDL-C decreased more significantly(P<0.05 or P<0.01).Whole blood low shear viscosity,whole blood high shear viscosity,plasma viscosity,whole blood reduction viscosity and fibrinogen of the treatment group decreased significantly(P<0.05 or P<0.01).Compared with the before treatment,the symptoms were improved significantly in treatment group(P<0.05 or P<0.01).Debilitation symptom of the data control group has improved more significantly(P<0.05).Conclusion Compound Hengqing decoction can reduce blood pressure(especially systolic blood pressure)in patients with hypertensive vertigo,ameliorate the symptoms,improve glucose and lipid metabolism and blood rhe-ology.And it is safe and effective on hypertensive vertigo.
