经皮椎间孔镜下髓核摘除术治疗腰椎间盘突出症的临床研究

Clinical Study on the Treatment of Lumbar Disc Herniation with Percutaneous Transforaminal Endoscopic Discectomy

目的:研究经皮椎间孔镜下髓核摘除术治疗腰椎间盘突出症的临床效果。方法:撷取本院收治的90例腰椎间盘突出症患者,撷取时间为2015年3月-2017年3月,分组原则以抽签法为主,分为观察组(n=45)和对照组(n=45)。对照组予以钉棒系统内固定治疗,观察组予以经皮椎间孔镜下髓核摘除术治疗。比较治疗效果、手术情况、VAS评分、JOA评分以及并发症发生率。结果:两组手术时间比较,差异无统计学意义(P>0.05);与对照组治疗效果比较,观察组较好(P<0.05);与对照组卧床活动时间比较,观察组较短(P<0.05);与对照组术中出血量比较,观察组较少(P<0.05),与对照组JOA评分比较,观察组较高(P<0.05);与对照组VAS评分以及并发症发生率比较,观察组均较低(P<0.05)。结论:经皮椎间孔镜下髓核摘除术可明显促进腰椎间盘突出症患者病情的改善,缓解疼痛,减少并发症,值得借鉴。

Objective:To study the clinical effect of percutaneous transforaminal endoscopic discectomy in the treatment of lumbar disc herniation.Method:90 patients with lumbar disc herniation admitted from March 2015 to March 2017 were divided into observation group(n=45)and control group(n=45).The control group was treated with screw fixation system,and the observation group was treated by percutaneous transforaminal endoscopic discectomy.The curative effect,operative condition,VAS score,JOA score and complication rate were compared.Result:There was no significant difference in the operation time between two groups(P>0.05).Compared with the control group,the effect of the observation group was better(P<0.05).Compared with the control group,the bed time of the observation group was shorter(P<0.05).Compared with the control group,blood loss of the observation group was less(P<0.05),and JOA score was highe than that of the control group(P<0.05).Compared with the control group,VAS score and the incidence of complications,the observation group were lower(P<0.05).Conclusion:Percutaneous transforaminal endoscopic discectomy can obviously promote the improvement of patients with lumbar disc herniation,relieve pain and reduce complications,which is worth learning from.
