三维适形放疗联合化疗与单纯三维适形放疗对食管癌的预后影响

Effect of Three-dimensional Conformal Radiotherapy Combined with Chemotherapy and Three-dimensional Conformal Radiotherapy on the Prognosis of Esophageal Cancer

目的:比较三维适形放疗(3D-CRT)联合化疗与单纯3D-CRT对食管癌患者的预后影响。方法:选取2010年7月-2014年5月本院肿瘤科收治的食管癌患者186例,根据患者所接受治疗方式的不同,分为单纯放疗组(单纯3D-CRT)和放化疗组(3D-CRT联合化疗),其中单纯放疗组89例,放化疗组97例,比较两组的短期疗效,不良反应发生率,1、3年局控率和生存率及生存质量。结果:放化疗组短期疗效,1、3年局控率和生存率及生存质量方面均高于单纯放疗组,差异均有统计学意义(P<0.05);放化疗组放射性食管炎、白细胞降低及恶心、呕吐方面高于单纯放疗组,差异均有统计学意义(P<0.05)。结论:3D-CRT联合化疗治疗食管癌疗效显著,短期疗效好,1、3年生存率高,生存质量好;放射性食管炎、白细胞降低及恶心、呕吐不良反应发生率增加,但可耐受可控制,值得临床推广普及。

Objective:To compare the effect of three-dimensional conformal radiotherapy(3D-CRT)combined with chemotherapy and 3D-CRT on the prognosis of patients with esophageal cancer.Method:From July 2010 to May 2014,186 patients with esophageal cancer treated in our hospital were selected.According to the different treatment methods,the patients were divided into radiotherapy alone group(3D-CRT,n=89)and radiotherapy and chemotherapy group(3D-CRT combined with chemotherapy,n=97).The short-term efficacy,the incidence of adverse reactions,1,3 years of local control rate,survival rate and quality of life of two groups were compared.Result:In the short-term curative effect,1,3 years of local control rate,survival rate and quality of life of radiotherapy and chemotherapy group were higher than those of radiotherapy alone group,the differences were statistically significant(P<0.05).The radiation esophagitis,leukopenia and nausea,vomiting of radiotherapy and chemotherapy group were higher than those of radiotherapy alone group,the differences were statistically significant(P<0.05).Conclusion:Three-dimensional conformal radiotherapy combined with chemotherapy for esophageal cancer is effective,short-term effect is good,1,3-year survival rate are high,has a good quality of life.Radioactive esophagitis,leukopenia and nausea,vomiting adverse reaction rate are increasing,but can be tolerable and control,it is worthy of clinical popularization.
