输尿管软镜与输尿管硬镜钬激光治疗输尿管上段结石对机体应激反应的影响

Effect of Ureteral Soft Mirror and Ureteral Hard Mirror Holmium Laser on the Stress Response of Body in the Treatment of Upper Ureteral Calculi

目的:探讨输尿管软镜与输尿管硬镜钬激光治疗输尿管上段结石对机体应激反应的影响。方法:选取2014年11月-2016年12月来本院医治的输尿管上段结石患者114例。根据治疗方法将其分为试验组(输尿管软镜下钬激光治疗)和对照组(输尿管硬镜下钬激光治疗),各57例。比较术前、术后第1、3、5天血清皮质醇(cortisol,Cor)、C反应蛋白(C-reactive protein,CRP)与白介素-6(interleukin-6,IL-6)水平。结果:治疗前,两组Cor、CRP与IL-6水平比较,差异均无统计学意义(P>0.05);术后第1、3、5天,两组Cor、CRP与IL-6水平均高于术前,但试验组Cor、CRP与IL-6水平均低于对照组,比较差异均有统计学意义(P<0.05)。结论:对输尿管上段结石患者而言,输尿管软镜钬激光治疗对身体所发生的免疫应激性反应少于输尿管硬镜,输尿管软镜钬激光治疗更符合当代泌尿外科实施微创型治疗的相关观念。

Objective:To investigate the effect of ureteral soft mirror and ureteral hard mirror holmium laser on the stress response of body in the treatment of upper ureteral calculi.Method:A total of 114 patients with upper ureteral calculi treated in our hospital from November 2014 to December 2016 were selected.According to the treatment methods,they were divided into experimental group(ureteral soft mirror holmium laser)and control group(ureteral hard mirror holmium laser),57 cases in each group.The serum levels of cortisol(Cor),C-reactive protein(CRP)and interleukin-6(IL-6)before and after operation first,third,fifth days between two groups were compared.Result:Before operation,the serum levels of Cor,CRP and IL-6 in two groups were compared,the differences were not statistically significant(P>0.05).After operation the first,third,fifth days,the serum levels of Cor,CRP and IL-6 in two groups were higher than those of before operation,but the serum levels of Cor,CRP and IL-6 in experimental group were lower than those of control group,the differences were statistically significant(P<0.05).Conclusion:For patients with upper ureteral calculi,the treatment of ureteral soft mirror holmium laser is less than ureteral hard mirror in the immune response to the body,the ureteral soft mirror holmium laser is more consistent with the concept of minimally invasive treatment in the contemporary department of urology.
