Roux-en-Y与Roux-en-Y加远端空肠储袋法应用在胃癌全胃切除患者中的预后对比分析

Comparison and Analysis the Prognosis between Roux-en-Y and Roux-en-Y Combined with Distal Jejunum Storage Bag Method in Gastric Cancer Total Gastrectomy

目的:对比分析Roux-en-Y与Roux-en-Y加远端空肠储袋法在胃癌全胃切除治疗中的应用效果。方法:选取本院收治的需接受全胃切除治疗的胃癌患者40例。按照随机数字表法将其分为对照组和观察组,各20例。对照组接受Roux-en-Y治疗,观察组接受Roux-en-Y加远端空肠储袋法治疗。比较两组手术相关情况、生存时间;比较两组术后生活质量评分(QLQ-STO22量表)。结果:两组手术时间、术中出血量、术后进食时间及下床时间比较,差异均无统计学意义(P>0.05)。两组术后1、6、12、24个月的QLQ-STO22评分均呈先降低后升高趋势(P<0.05);术后1 d、1个月,两组QLQ-STO22评分比较差异均无统计学意义(P>0.05);术后6、12、24个月,观察组QLQ-STO22评分均低于对照组,比较差异均有统计学意义(P<0.05)。在24个月的随访过程中,观察组生存期为(22.02±2.37)个月,高于对照组的(15.52±3.95)个月(t=6.310,P=0.000)。结论:与Roux-en-Y相比,Roux-en-Y加远端空肠储袋法可有效地改善胃癌全胃切除的临床治疗效果。

Objective:To compare and analyze the effect of Roux-en-Y and Roux-en-Y combined with distal jejunum storage bag method in gastric cancer total gastrectomy.Method:A total of 40 patients with gastric cancer who were treated with total gastrectomy in our hospital were selected.According to the random number table method,they were divided into control group and observation group,20 cases in each group.The control group received Roux-en-Y treatment,and observation group received Roux-en-Y combined with distal jejunum storage bag method treatment.The operation related conditions and survival time of two groups were compared,and quality of life score(QLQ-STO22 scale)after operation of two groups were compared.Result:The operation time,intraoperative bleeding,postoperative eating time and out of bed time in two groups were compared,the differences were not statistically significant(P>0.05).After operation 1,6,12,24 months,the QLQ-STO22 scores in two groups were decreased first and then increased(P<0.05),after operation 1 d and 1 month,the QLQ-STO22 scores in two groups were compared,the differences were not statistically significant(P>0.05),after operation 6,12,24 months,the QLQ-STO22 score of observation group were lower than those of control group,the differences were statistically significant(P<0.05).During the follow-up period of 24 months,the survival period of observation group was(22.02±2.37)months,which was higher than(15.52±3.95)months of control group(t=6.310,P=0.000).Conclusion:Compared with Roux-en-Y,Roux-en-Y combined with distal jejunum storage bag method can effectively improve the clinical effect of total gastrectomy of gastric cancer.
