摘要
目的:探讨血液透析滤过、高通量血液透析应用于慢性肾衰竭尿毒症患者治疗临床效果。方法:选取2016年3月-2017年4月本院收治的慢性肾衰竭尿毒症患者82例。按照随机数字表法将其分为观察组(血液透析滤过)、对照组(高通量血液透析),各41例。比较两组临床疗效、并发症、治疗前后的血肌酐水平、免疫球蛋白水平及肺功能指标。结果:观察组治疗总有效率为97.56%,高于对照组的82.93%(x^2=4.986,P=0.025),观察组并发症发生率14.63%,低于对照组的41.46%(x^2=7.312,P=0.007);治疗后,两组血肌酐、RV水平均低于治疗前,IgM、Ig A、IgG、FVC、VC、TLC水平均高于治疗前,且观察组血肌酐、RV水平低于对照组,Ig M、IgA、IgG、FVC、VC、TLC水平均高于对照组,比较差异均有统计学意义(P<0.05)。结论:血液透析滤过、高通量血液透析应用于慢性肾衰竭尿毒症治疗,均可使患者免疫功能得到有效提高,促进患者肺功能得到有效改善,但与血液透析滤过相比,高通量血液透析治疗效果更加显著。
Objective:To investigate the clinical effect of high throughput hemodialysis and hemodialysis filtration in chronic renal failure uremia.Method:A total of 82 patients with chronic renal failure uremia treated in our hospital from March 2016 to April 2017 were selected,according to the random number table method,they were divided intoobservation group(hemodialysis filtration)and control group(high throughput hemodialysis),41 cases in each group.The clinical effects,complications,serum creatinine level,immunoglobulin level and lung function index before and after treatment between two groups were compared.Result:The total effective rate of treatment in observation group was 97.56%,which was higher than 82.93%of control group(Χ2=4.986,P=0.025).The incidence of complications in observation group was 14.63%,which was lower than 41.46%of control group(Χ2=7.312,P=0.007).After treatment,the levels of serum creatinine and RV in two groups were all lower than those of before treatment,the levels of IgM,IgA,IgG,FVC,VC and TLC were all higher than those of before treatment,and the levels of serum creatinine and RV in observation group were lower than those of control group,the levels of IgM,IgA,IgG,FVC,VC and TLC were all higher than those of control group,the differences were statistically significant(P<0.05).Conclusion:High throughput hemodialysis and hemodialysis filtration in treatment of chronic renal failure uremia can effectively improve the immune function of patients and promote the effective improvement of pulmonary function,but compared with hemodialysis filtration,the effect of high throughput hemodialysis is more remarkable.
