康柏西普和曲安奈德行玻璃体腔注射治疗糖尿病视网膜病变的效果对比

Comparison of the Effect of Intravitreal Injection of Conbercept and Triamcinolone Acetonide on Diabetic Retinopathy

目的:比较在玻璃体腔内注射康柏西普和曲安奈德治疗糖尿病视网膜病变(diabetic retinopathy,DR)的临床疗效及安全性。方法:选取2014年1月-2017年6月本院收治的DR患者119例,按照入院先后顺序分为对照组59例和观察组60例。对照组给予曲安奈德注射,观察组给予康柏西普注射。比较两组治疗前后的戴眼镜后视力、血管内皮生长因子水平、视网膜厚度及治疗后的生活质量及并发症。结果:治疗后1、3个月,两组戴眼镜后视力均高于治疗前,视网膜厚度和血管内皮生长因子水平均低于治疗前,且观察组各指标均优于对照组,比较差异均有统计学意义(P<0.05);治疗后3个月,两组戴眼镜后视力均高于治疗后1个月,视网膜厚度和血管内皮生长因子水平均低于治疗后1个月,比较差异均有统计学意义(P<0.05);治疗后3个月,观察组活动能力、自理情况、社交与心理评分均高于对照组,前房炎性反应、角膜发炎和眼压升高的发生率及总发生率均低于对照组,比较差异均有统计学意义(P<0.05)。结论:玻璃体腔内注射康柏西普相较于注射曲安奈德可以更有效地改善DR患者的视网膜功能、视力和生活质量,且安全性更高,对提高患者生活质量有积极的作用,值得临床推广。

Objective:To compare the clinical efficacy and safety of intravitreal injection of Conbercept and Triamcinolone Acetonide in treatment of diabetic retinopathy(DR).Method:A total of 119 patients with DR in our hospital from January 2014 to June 2017 were selected.According to the order of admission,they were divided into 59 cases of control group and 60 of observation group.The control group was given Triamcinolone Acetonide injection,and observation group was given Conbercept injection.The visual acuity after wearing glasses,vascular endothelial growth factor,retinal thickness before and after treatment,and quality of life and complications after treatment were compared between two groups.Result:After treatment 1 and 3 months,the visual acuity after wearing glasses in two groups were higher than those of before treatment,the vascular endothelial growth factor and retinal thickness were lower than those of before treatment,and all the indexes of observation group were better than those of control group,the differences were statistically significant(P<0.05).After treatment 3 months,the visual acuity after wearing glasses in two groups were higher than that of after treatment 1 month,the vascular endothelial growth factor and retinal thickness were lower than those of after treatment 1 month,the differences were statistically significant(P<0.05).After treatment 3 months,the activity,self-care,social and psychological scores in observation group were higher than those of control group,the total incidence of complications and incidence of anterior chamber inflammation,corneal inflammation and intraocular pressure were lower than those of control group,the differences were statistically significant(P<0.05).Conclusion:Compared with Triamcinolone Acetonide,intravitreal injection of Conbercept can improve retinal function,vision and quality of life of DR patients more effectively,has higher safety,and has a positive effect on improving the quality of life of patients,which is worthy of clinical promotion.
