内镜下构建抗反流动物的实验研究 被引量:1

摘要 目的:探讨内镜下构建胃食管抗反流动物模型的可行性。方法:选取6~12月龄胃食管反流病的Beagle实验犬模型12只,分别在内镜下构建抗反流动物模型,另选10只正常犬作为对照,比较各组犬食管压力、食管p H和食管病理学检查情况。结果:12只胃食管反流病实验犬构建抗反流动物模型中,10例构建成功,1例构建失败,1例由于吻合口瘘死亡,成功率为83.3%;反流犬食管压力明显低于正常犬和抗反流犬,差异均有统计学意义(t=4.457、4.316,P<0.05),而抗反流犬与正常犬食管压力比较差异无统计学意义(t=0.691,P>0.05);反流犬、抗反流犬和正常犬反流>5 min次数比较,差异无统计学意义(P>0.05),反流犬的反流次数和反流最长时间均明显高于正常犬和抗反流犬(t=5.094、5.515,P<0.05;t=4.028、4.846,P<0.05),而抗反流犬和正常犬相比差异均无统计学意义(t=1.361、1.845,P>0.05);反流犬和抗反流犬的反流率均明显高于正常犬(t=8.416、4.337,P<0.05),反流犬的反流率也明显高于抗反流犬(t=5.217,P<0.05);正常实验犬食管组织呈现明显的复层鳞状结构,上皮层内有少量炎症细胞存在,其他各层均正常;胃食管反流实验犬食管组织中上皮层存在明显的膨胀和增厚,黏膜层有大量的炎症细胞存在,此外部分肌层还存在断裂等情况;胃食管抗反流实验犬食管组织中上皮层有轻微膨胀或增厚,黏膜层有较多炎症细胞存在,未发现基层断裂现象。结论:采用本研究方法构建的抗反流动物抗反流效果显著,可用于抗反流动物构建的相关实验研究。 Objective:To explore the feasibility of structuring gastroesophageal anti-reflux animal model with endoscope.Method:Twelve 6-12-month-old Beagle experiment dogs which have gastroesophageal reflux are separately structured gastroesophageal reflux animal model with endoscope.Ten more normal dogs were selected as comparison which includes esophageal pressure,esophagus pH and histopathological observation of esophagus.Result:In 12 dogs with gastroesophageal reflux,10 dogs were successfully structured and 1 dog was failed,1 dog died of anastomotic fistula,the success rate was 83.3%.The esophageal pressure of reflux dogs was significantly lower than those of normal dogs and anti reflux dogs,the differences were statistically significant(t=4.457,4.316,P<0.05),while the esophageal pressure of anti reflux dogs and normal dogs had no significant difference(t=0.691,P>0.05).There was no significant difference in the times of reflux more than 5 min among three dogs(P>0.05).Reflux times and the longest time of reflux of reflux dogs were significantly higher than those of normal dogs and anti reflux dogs(t=5.094,5.515,P<0.05;t=4.028,4.846,P<0.05),while anti reflux dogs and normal dogs had no statistical difference(t=1.361,1.845,P>0.05).The reflux rate of anti reflux dogs and anti reflux dogs were significantly higher than those of normal dogs(t=8.416,4.337,P<0.05),the reflux rate of reflux dogs was significantly higher than that of anti reflux dogs(t=5.217,P<0.05).The esophageal tissue of normal experiment dog showed a significant layer of squamous structure.Small amount of inflammatory cells existed in the epithelium.Other layers showed normal.The middle and up dermic layer of experiment dogs appeared significant expansion and thickening.Plenty of inflammatory cells existed in mucosal layer.In addition,partial muscular layers showed rupture and so on.The middle and up dermic layer of antireflux dogs’esophageal tissue had a slight expansion or thickening.There were more inflammatory cells and no grassroots fracture was found.Conclusion:The anti-reflux effect of anti-reflux animal model structure which adopts present research method is remarkable.It can be applied into relevant experiment and study of anti-reflux animal model structure.
