对比分析区域性腹膜后减压低位引流与开腹引流术治疗急性重症胰腺炎的临床效果

Comparative Analysis of Clinical Effect of Regional Retroperitoneal Decompression and Low Drainage and Open Drainage in Treatment of Severe Acute Pancreatitis

目的:对比分析区域性腹膜后减压低位引流与开腹引流术治疗急性重症胰腺炎的临床效果。方法:本次60例研究对象均选自本院2014年10月-2016年10月期间收治的急性重症胰腺炎患者,根据随机法将患者分为对照组和分析组,每组各30例。其中分析组选择区域性腹膜后减压低位引流,对照组选择开腹引流术治疗,比较两组患者的并发症发生情况、炎症反应变化和手术指标变化情况。结果:两组患者术后各项并发症发生率比较差异均无统计学意义(P>0.05)。两组患者术前的IL-6、CRP和TNF-α水平比较差异均无统计学意义(P>0.05);分析组患者术后14 d的IL-6、CRP和TNF-α水平均低于对照组,两组比较差异均有统计学意义(P<0.05)。分析组患者的手术时间、住院时间和住院经费均低于对照组,两组比较差异均有统计学意义(P<0.05)。结论:针对急性重症胰腺炎患者而言,选择区域性腹膜后减压低位引流治疗的效果甚佳,可减轻患者炎症反应,减少患者手术时间和住院时间,降低治疗经费,值得推广。

Objective:To compare and analyze the clinical effects of regional retroperitoneal decompression and low drainage and open drainage in treatment of severe acute pancreatitis.Method:60 cases with acute severe pancreatitis and who accepted treatments in our hospital from October 2014 to October 2016 were selected as the research objects,and the patients were divided into the control group and the analysis group according to random method,30 cases in each group.The analysis group was given regional retroperitoneal decompression and low drainage,while the control group was treated with open drainage.Then,the complications,inflammatory reaction and operation index of two groups were compared.Result:There was no significant difference in the incidence of complications after the operation between two groups(P>0.05).There was no significant difference in the levels of IL-6,CRP and TNF-αbefore the operation between the two groups(P>0.05).The levels of IL-6,CRP and TNF-αafter the operation for 14 days in the analysis group were lower than those of the control group,the differences were statistically significant(P<0.05).The operation time,hospitalization time and hospitalization expenses of the patients in the analysis group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:For patients with severe acute pancreatitis,the treatment effects of regional retroperitoneal decompression and low drainage are good,which can reduce the inflammatory reaction of patients,reduce the operation time and the hospitalization time of patients,and reduce the treatment funds,thus it is worth promoting.
