超声引导下中心静脉置管对重症监护室心肺复苏老年患者的疗效

The effect and safety of ultrasound-guided central venous catheterization in elderly patients with cardiopulmonary resuscitation in ICU

目的探讨对重症监护室(ICU)心肺复苏老年患者治疗中行超声引导下中心静脉置管的疗效及其安全性。方法选取2013年9月至2016年12月该院接受治疗的80例心肺复苏老年患者作为研究对象,将其随机分为观察组和对照组,每组各40例。观察组患者行超声引导下中心静脉置管予以治疗,而对照组患者则行常规穿刺中心静脉置管予以治疗。比较两组患者的手术情况、穿刺成功率及并发症发生情况。结果观察组患者的手术时间、穿刺探查时间以及术中出血量均明显少于对照组,差异有统计学意义(P<0.05);观察组患者1次穿刺成功率明显高于对照组,差异有统计学意义(P<0.05);观察组患者并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。结论对于ICU心肺复苏老年患者,超声引导下中心静脉置管能够获得比常规穿刺置管更好的临床效果,有效提高穿刺成功率,减少并发症的发生,具有重要的临床意义。

Objective To investigate the effect and safety of ultrasound-guided central venous catheterization in elderly patients with cardiopulmonary resuscitation in ICU.Methods A total of 80 cases of elderly patients with cardiopulmonary resuscitation in the hospital were selected from September 2013 to December 2016,they were randomly divided into the observation group(40 cases)and control group(40 cases).The patients of control group were treated with conventional puncture center venous catheterization,while the patients of observation group were treated with ultrasound-guided central venous catheterization.The surgery,puncture success rate and complications of two groups were compared.Results The operation time,puncture time and intraoperative blood loss in the observation group was significantly less than that in the control group,difference was statistically significant(P<0.05).The one intubation success rate in the observation group was significantly higher than that in the control group,difference was statistically significant(P<0.05).And the incidence of complications in the observation group was significantly lower than that in the control group,difference was statistically significant(P<0.05).Conclusion Compared with conventional puncture catheter,ICU elderly patients with cardiopulmonary resuscitation underwent ultrasound-guided central venous catheterization can achieve better clinical results,improve the success rate of puncture effectively,and reduce the incidence of complications.Ultrasound-guided central venous catheterization in ICU elderly patients with cardiopulmonary resuscitation is of important clinical significance.
