接骨药丸联合PRP治疗股骨颈骨折的疗效观察

Jieguyao pills combined with platelet-rich plasma in treatment of femoral neck fracture

目的 观察接骨药丸联合自体富血小板血浆（PRP）治疗股骨颈骨折术后的临床疗效。方法 选取2015年1月至2016年12月在本院治疗的股骨颈骨折患者90例，按随机数字表法随机分为3组，每组30例。均采用闭合复位空心螺钉内固定手术后，A组采用接骨药治疗，B组采用PRP治疗，C组采用接骨药联合PRP治疗。定期随访记录3组患者的骨折不愈合率、股骨头坏死率及Harris髋关节功能评分。结果 C组术后骨折不愈合率和股骨头坏死率（3.33%、3.33%）均明显低于A组（26.67%、30.00%）和B组（26.67%、26.67%），差异均有统计学意义（均P＜0.05）；C组患者术后9个月Harris髋关节功能评分均值及优良率[（86.67±7.59）分，86.67%）]均明显高于A组[（78.20±11.37）分，53.33%）]和B组[（80.27±9.73）分，63.33%）]，差异均有统计学意义（均P＜0.05）。结论 股骨颈骨折闭合复位内固定术后联用接骨药丸与PRP治疗，能明显促进骨折愈合，减少并发症，提高髋关节功能，改善患者生活质量。

Objective To observe the clinical efficacy of Jieguyao pills combined with autologous platelet-rich plasma(PRP)in the treatment of femoral neck fracture.Methods From Jan.2015 to Dec.2016,90 patients with femoral neck fracture were randomly divided into three groups,30 cases for each group.After being treated with closed reduction and hollow screw internal fixation,group A were treated with jieguyao pills,group B with PRP,and group C with Jieguyao pills and PRP.The fracture nonunion rate,femoral head necrosis rate,and Harris hip function score in the three groups were followed up and recorded.Results After the operation,the rates of nonunion and femoral head necrosiswere significantly lower in group C(3.33%and 3.33%)than in group A(26.67%and 30.00%)and group B(26.67%and 26.67%),with statistical differences(all P<0.05).Nine months the after operation,the Harris hip score and excellent rate were significantly higher in group C[(86.67±7.59)and 86.67%]than in group A[(78.20±11.37)and 53.33%]and group B[(80.27±9.73)and 63.33%],with statistical differences(all P<0.05).Conclusion After closed reduction and internal fixation for femoral neck fracture,the combination of Jieguyao pills and PRP can obviously promote the fracture healing,reduce complications,and improve the patients’function of hip joint and the quality of life.
