自制简易定位器在经皮椎弓根穿刺技术中的应用 被引量:4

摘要 目的:探讨自制的简易定位器在经皮椎弓根穿刺技术中的临床应用效果。方法:选取接受经皮椎体成形术或经皮椎体后凸成形术的骨质疏松性椎体压缩骨折患者62例,其中32例使用自制简易定位器进行术前定位,作为A组。30例为以往使用传统网格状定位板进行术前定位,作为B组。选取接受经皮椎弓根螺钉内固定手术的胸腰椎暴力骨折患者65例,其中32例使用自制简易定位器进行术前定位,作为C组。33例为以往使用传统网格状定位板进行术前定位,作为D组。比较A、B组间,C、D组间在透视次数、穿刺时间上的区别。结果:透视次数方面A、C两组明显少于B、D两组(P<0.05),穿刺时间方面A、C两组明显少于B、D两组(P<0.05)。结论:采用自制简易定位器进行经皮椎弓根穿刺,可显著降低术中透视次数及穿刺时间,大大减少了患者及医护人员接触的放射量,缩短了手术时间,提高了手术安全性。 Objective:To discuss the clinical effect of self-made positioner in percutaneous transpedicular puncture.Methods:62 patients with osteoporotic vertebral compression fractures underwent PVP or PKP were chosen.Among them,32 patients'preoperative localization were used by self-made simple positioner,as A group.30 patients'preoperative localization were used by previous grid positioning plate,as B group.And 65 patients with thoracolumbar fractures underwent percutaneous pedicle screw placement were chosen.Among them,32 patients'preoperative localization were used by self-made simple positioner,as C group.33 patients'preoperative localization were used by previous grid positioning plate,as D group.The differences between group A and B,group C and D,and the aspects of X-ray fluoroscopy times and puncture time were compared.Results:The patients in A and C group had fewer Xray fluoroscopy times and puncture time than B and D group(P<0.05).Conclusion:Percutaneous transpedicular puncture with selfmade positioner can reduce X-ray fluoroscopy times and puncture time obviously,which reduces the amount of radiation exposure to patients and health care providers,shortens the operation time and improves the operation safety.
