盐酸利托君与硫酸镁治疗先兆早产的效果及不良反应探讨 被引量:7

摘要 目的探讨盐酸利托君与硫酸镁治疗先兆早产的效果及不良反应。方法将112例先兆早产患者按治疗方法不同分为盐酸利托君组(n=62)与硫酸镁组(n=50),比较两组药物起效时间、延长妊娠时间及保胎情况;治疗前后血清前列腺素E2(PGE2)、一氧化氮(NO)、白介素-2(IL-2)、IL-6水平以及宫颈分泌物胎儿纤连蛋白(FFN)、基质金属蛋白酶-2(MMP-2)、MMP-9、磷酸化胰岛素样生长因子-1(IGFBP-1)水平;不良反应发生情况。结果盐酸利托君组药物起效时间明显短于硫酸镁组(P<0.05),延长妊娠时间明显长于硫酸镁组(P<0.05),保胎成功率明显高于硫酸镁组(P<0.05)。两组治疗后血清PGE2、NO、IL-2、IL-6及FFN、MMP-2、MMP-9、IGFBP-1水平均明显下降(P<0.05),盐酸利托君组明显低于硫酸镁组(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论盐酸利托君治疗先兆早产安全有效,疗效优于硫酸镁。 Objective To investigate the effect and adverse effects of ritodrine hydrochloride and magnesium sulfate in the treatment of threatened preterm labor.Methods One hundred and twelve patients with threatened preterm labor were divided into two groups according to different treatment methods:ritodrine hydrochloride group(n=62)and magnesium sulfate group(n=50).The time of drug onset,the prolongation of pregnancy time and the condition of fetal preservation were compared between the two groups.Before and after treatment,serum prostaglandin E2(PGE2),nitric oxide(no),Interleukin-2(IL-2),IL-6,Fetal fibronectin(FFN),matrix metalloproteinase(MMP-2),MMP-9,Phosphorylated insulin-like growth factor-1(IGFBP-1),and the occurrence of adverse reactions were observed.Results The onset time of ritodrine hydrochloride group was significantly shorter than that of magnesium sulfate group(P<0.05),and the prolonged pregnancy time was significantly longer than that of magnesium sulfate group(P<0.05).The successful rate of fetal preservation was significantly higher than that of magnesium sulfate group(P<0.05).After treatment,the levels of PGE2,NO,IL-2,IL-6,FFN,MMP-2,MMP-9,IGFBP-1 in serum of the two groups were significantly decreased(P<0.05),but the incidence of adverse reactions was significantly lower in the ritodrine hydrochloride group than in the magnesium sulfate group(P<0.05),and there was no significant difference between the two groups in the incidence of adverse reactions.Conclusion Ritodrine hydrochloride is safe and effective in the treatment of threatened premature labor,and the curative effect is better than that of magnesium sulfate.Ritodrine hydrochloride is safe and effective in the treatment of threatened preterm labor,and the curative effect is better than that of magnesium sulfate.
