海尔曼螺杆菌与幽门螺杆菌相关性胃炎的临床病理特征分析

Analysis of clinicopathological features of Helicobacter Heilman and Helicobacter pylori associated gastritis

目的探析海尔曼螺杆菌相关性胃炎与幽门螺杆菌相关性胃炎的临床病理特征及差异。方法选取2012年8月~2017年8月在我院接受胃镜和病理检查的6900例患者为主要研究对象,对所有患者的胃镜和病理检查结果进行回顾性分析。结果海尔曼螺杆菌感染的检出率为0.637%,明显低于幽门螺杆菌感染的检出率(33.73%);海尔曼螺杆菌相关性胃炎中的螺杆菌定植量明显少于幽门螺杆菌感染病例,且胃黏膜急性炎症程度和慢性炎症程度相对较轻,经内镜检查可见的溃疡性病变数量少于幽门螺杆菌感染患者(P<0.05);两组病例中肠上皮化生、上皮内瘤变、腺体萎缩的检出率和病变程度比较,差异无统计学意义(P>0.05)。结论海尔曼螺杆菌相关性胃炎在临床上的发病率非常低,此类胃炎的炎症反应相对较轻,但癌前病变的检出率和病变程度与幽门螺杆菌相关性胃炎相似,在胃部恶性疾病的进展过程中,海尔曼螺杆菌的作用可能与幽门螺杆菌相类似。

Objective To explore the clinicopathological features and differences of Helicobacter Heilman associated gastritis and Helicobacter pylori associated gastritis.Methods 6900 patients who received gastroscopy and pathological examination in our hospital from August 2012 to August 2017 were selected as the main subjects.The gastroscopy and pathological examination results of all the patients were retrospectively analyzed.Results The detection rate of Helicobacter Heilman infection was 0.637%,which was significantly lower than that of Helicobacter pylori infection(33.73%).The colonization of Helicobacter in Helicobacter Heilman associated gastritis was significantly less than that of Helicobacter pylori infection,and the degree of acute inflammation and chronic inflammation of gastric mucosa were relatively mild,the number of ulcerative lesions seen by endoscopic examination was less than that of Helicobacter pylori infection(P<0.05).There was no significant difference in the detection rate and degree of intestinal metaplasia,intraepithelial neoplasia and gland atrophy in the two groups(P>0.05).Conclusion The incidence of Heilman associated gastritis in the clinic is very low,and the inflammation of this kind of gastritis is relatively mild.However,the detection rate and degree of precancerous lesions were similar to those of Helicobacter pylori associated gastritis.In the progression of malignant stomach disease,the role of Helicobacter Heilman may be similar to that of Helicobacter pylori.
