黄连软膏联合心理干预对肛周脓肿患者术后恢复情况、疼痛及情绪的影响

Effect of Huanglian Ointment Combined with Psychological Intervention on Postoperative Recovery,Pain and Emotion in Patients with Perianal Abscess

目的探讨黄连软膏联合心理干预对肛周脓肿患者术后恢复情况、疼痛及情绪的影响。方法选择医院2012年1月至2017年4月收治的肛周脓肿术后患者46例,采用随机数字表法分为观察组和对照组,各23例。两组患者均术后静脉滴注氨曲南,术后3 d开始换药,采用碘伏反复消毒脓腔,消毒后给脓腔内涂抹黄连软膏并填塞纱布,隔日换药至创面上皮组织完全生长。观察组在此基础上联合心理干预。结果术后5,7,14,21 d时两组患者创面愈合率均逐渐明显提高(P<0.05),且观察组患者术后7,14 d时创面愈合率明显高于对照组(P<0.05);观察组患者脓液消失时间及创面水肿消失时间均明显短于对照组患者(P<0.05);术后3,5,7,14 d,两组患者疼痛视觉模拟(VAS)评分、焦虑自评量表(SAS)评分均逐渐明显下降(P<0.05),且观察组患者术后3,5,7 d VAS评分均明显低于对照组患者(P<0.05),观察组患者各时间点SAS评分均明显低于对照组患者(P<0.05);对照组发生1例伤口出血,2例伤口感染,观察组发生2例伤口感染,两组患者术后并发症及不良反应发生率相近(P>0.05)。结论黄连软膏联合心理干预对肛周脓肿患者术后康复疗效肯定,可帮助患者减轻疼痛感,缓解不良情绪,且不良反应较轻微,值得临床推广。

Objective To investigate the effect of Huanglian Ointment combined with psychological intervention on postoperative recovery,pain and mood in patients with perianal abscess.Methods Totally 46 patients with perianal abscess admitted to our hospital from January 2012 to April 2017 were randomly divided into the observation group and the control group by random number table method,23 cases in each group.The two groups were given intravenous drip of aztreonam after operation,and dressing change from 3d after operation,the abscess was repeatedly sterilized with iodophor,after disinfection,the abscess was smeared with Huanglian Ointment and filled with gauze.The patiens were given dressing change every other day until the epithelial tissue of the wound was fully grown.On this basis,the observation group was given psychological intervention.Results The healing rate of wounds in the two groups increased gradually on 5th,7th,14th,21st d after operation(P<0.05),and the healing rates of wounds in the observation group were significantly higher than those in the control group on 7th,14th d after operation(P<0.05).The disappearance time of the pus and wound edema in the observation group was significantly shorter than that of the control group(P<0.05).The VAS scores and SAS scores of the two groups gradually decreased in the two groups on 3rd,5th,7th,14th d after operation(P<0.05),and the VAS scores in the observation group were lower than those in the control group on the 3rd,5th,7th d after operation,the SAS score at each time point after operation was significantly lower than that of the control group(P<0.05).The control group had 1 case of wound bleeding,2 cases of wound infection,and the observation group had 2 cases of wound infection,there was no significant difference in the incidence rate of postoperative complications and adverse reactions between the two groups(P>0.05).Conclusion Huanglian Ointment combined with psychological intervention has a certain effect on postoperative recovery of patients with perianal abscess,it can help patients relieve pain and alleviate bad emotions,and it has mild adverse reaction,which is worthy of clinical promotion.
