产前超声诊断右室双出口12例临床分析

Clinical Analysis of 12 Cases of Right Ventricular Double Exit Diagnosed by Prenatal Ultrasound

目的分析12例胎儿右室双出口超声声像图特征,探讨右室双出口的病理分型、鉴别诊断及预后,提高对该病的认识。方法选择2010年1月—2017年5月在云南省第二人民医院行产前超声检查的32 645例胎儿超声资料进行回顾性分析,对胎儿进行胎儿心脏畸形筛查,对12例右室双出口超声声像图特点进行总结和分析。结果 2例为法洛四联症型右室双出口,均术后证实;4例为Taussing-bing畸形,其中3例因合并其他畸形引产后病理检查证实、1例出生后心脏彩超复查后证实;1例为主动脉左转位型右室双出口合并其他畸形,失访;5例为经典型右室双出口,均合并其他畸形,其中2例染色体异常,引产后病理检查证实。结论产前超声对诊断胎儿右室双出口有非常重要的价值,对评估胎儿预后和产前咨询有重要的临床指导意义。

Objective Analyze the characteristics of the double export sound image in the right ventricle of the fetus,to explore the pathologic classification,differential diagnosis and prognosis of the right ventricular double outlet,and to improve the understanding of the disease.Methods From January 2010 to May 2017 in Yunnan Second People's Hospital to prenatal ultrasound examination of 32,645 cases of fetal data were analyzed retrospectively,to all screening for fetal cardiac malformation fetus,12 cases of double outlet of right ventricular ultrasonic ultrasonographic features were concluded and analyzed.Results TWO cases were confirmed after surgery in the right ventricle double outlet of falo.FOUR cases were deformity of Taussing-bing,3 patients were confirmed by pathological examination after the combination of other deformities,and 1 case was confirmed after the postnatal examination.One case of left-right ventricular double exit of main artery was combined with other deformities,missing visit;5 cases were double exportation of the typical right ventricle,and 5 cases were combined with other deformities,including 2 cases of chromosome abnormality,and pathologic confirmation after induced birth.Conclusion Prenatal ultrasonography has important value in diagnosis of fetal right ventricular double outlet,and it has important clinical significance for evaluating fetal prognosis and prenatal counseling.
