摘要
目的通过预防性及细致化操作,减少乳腺X线摄影三维钩丝定位操作的辐射剂量及并发症。方法收集2014年12月~2016年9月期间在我院行X线摄影三维钩丝定位的女性患者34例,按随机分组原则均分为低剂量组与正常剂量组,分别记录两组在X线摄影三维钩丝定位操作中的平均腺体剂量(AGD)值与入射皮肤剂量(ESD)值,分析34例患者并发症出现原因及预防方法。结果 34例患者均定位成功,穿刺针距离穿刺中心均<2cm,两组患者钩丝定位AGD与ESD数值差异有统计学意义(P<0.05),穿刺针距离穿刺中心长度差异无统计学意义(P>0.05),术中出现迷走神经反应5例,定位软件计算错误3例,钩丝移位扭曲4例。结论在乳腺X线摄影三维钩丝定位过程中,通过改变摄片条件及细致化操作,可有效降低辐射剂量及并发症发生率。
Objective To reduce the radiation dose and complication of breast X-ray three-dimensional hook wire localization by preventive and meticulous operation.Methods 34 female patients with X-ray three-dimensional hook wire were collected from December 2014 to September 2016.The patients were divided into low dose group and normal dose group according to the principle of random grouping.AGD values and incident skin dose(ESD)values in the three-dimensional hook-wire positioning operation were used to analyze the causes and prophylaxis of complications in 34 patients.Results There were statistically significant differences in the AGD and ESD values between the two groups(P<0.05).There was no significant difference in the length of the puncture needle distance between the two groups(P>0.05).There were 5 cases of vagal reaction in the operation,3 cases of misplaced software and 4 cases of distortions of the hook and wire.Conclusion In the process of positioning the three-dimensional hook wire of the mammary gland,by changing the radiographic condition and meticulous operation can effectively reduce the radiation dose and the incidence of complications.
