MSCT肺动脉造影对肺栓塞患者诊断及对患者右心功能评价价值分析

Value of multi-slice spiral CT pulmonary angiography in judging the severity of pulmonary embolism and evaluating the right ventricular function in patients with pulmonary embolism

目的探究多层螺旋CT肺动脉造影对肺栓塞患者疾病严重程度判断及分析其对患者右心功能评价价值。方法选择我院收治的127例确证为肺栓塞的患者及51例健康者作为研究对象,比较四组的右心室短轴最大径(RVMSA)、左心室短轴最大径(LVMSA)、右心室/左心室短轴最大径(RV/LV)、肺动脉直径、上腔静脉直径。比较肺栓塞三组的CT阻塞指数、支气管动脉扩张例数、室间隔平直或左突例数。结果四组的RV、LV、RV/LV、肺动脉直径、上腔静脉直径,各项指标均有明显差异(P<0.05)。三组的CT阻塞指数、支气管动脉扩张例数、室间隔平直或左突例数,各项指标均有明显差异,重度组患者的CT阻塞指数最高、支气管动脉扩张例数及室间隔平直或左突例数明显多于其他两组(P<0.05)。结论肺栓塞患者的RV、LV、RV/LV、肺动脉直径、上腔静脉直径可作为判断肺栓塞严重程度的指标,多层螺旋CT肺动脉造影用于评价患者肺栓塞严重程度及右心功能具有重要应用价值。

Objective To explore the value of multi-slice spiral CT pulmonary angiography in judging the severity of pulmonary embolism in patients with pulmonary embolism and to analyze its value in evaluating the right ventricular function.Methods 127 patients with pulmary emholism,who were admitted to our hospital and 51 healty subjects were ineluded in this work.The right ventricular short axis diameter(RVMSA),left ventricular short axis diameter(LVMSA),right ventricular and left ventricular short axis diameter(RV/LV),pulmonary artery diameter,vena cav diameter were compared in four gorups.The CT occlusion index,the number of bronchial artery dilatation,the interventricular septum straightness,or the number of left protrusion were compared between the three groups.Results There were significant differences in RV,LV,RV/LV,pulmonary artery diameter,superior vena cava diameter between the four groups(P<0.05).The three group of CT obstruction index,the number of cases of bronchial artery dilatation,left ventricular septal straight or the number of cases,the indicators were significantly different in patients with severe CT obstruction index,the highest number of cases of bronchial artery dilatation and left ventricular septal straight or the number of cases were significantly more than the other two groups(P<0.05).Conclusion Pulmonary embolism in patients with RV,LV,RV/LV,pulmonary artery diameter,superior vena cava diameter can be used to determine the severity of pulmonary embolism.CT pulmonary artery angiography for patients with pulmonary embolism severity evaluation of right ventricular function has important application value.
