乌拉地尔治疗高血压心力衰竭患者48例临床观察

Chinical Observation of Urapitil on 48 cases of Cardiac Failure with Hypertension

目的了解乌拉地尔联合常规治疗对高血压心力衰竭患者心功能的影响。方法选取96例高血压伴心力衰竭患者为受试对象,依照随机数表分为观察组与对照组,各48例。对照组予以常规三药联合(地高辛片+螺内酯片+呋塞米片)方案治疗,观察组在此基础上予以乌拉地尔注射液进行治疗。比较两组患者治疗前及治疗后第7天血压指标[收缩压(SBP)、舒张压(DBP)]、心功能指标[心率(HR)、左室射血分数(LVEF)]及血清N末端脑钠肽前体(NT-pro BNP)水平差异,观察治疗后1个月内药物不良反应(头痛、心悸、面色潮红、消化系统紊乱等)发生情况。结果治疗后第7天,两组患者血压(SBP、DBP)水平较治疗前均降低,差异有统计学意义(P<0.05),但组间比较差异无统计学意义(P>0.05);观察组患者HR、血清NT-pro BNP水平均低于对照组,差异有统计学意义(P<0.05);观察组LVEF水平升高,明显高于对照组,差异有统计学意义(P<0.05)。治疗后1个月内,两组患者药物不良反应发生率比较,差异无统计学意义(P>0.05)。结论乌拉地尔联合常规治疗可有效改善高血压伴心力衰竭患者的心功能。

Objective To study the curative effects of urapidil combined with conventional therapy in improving the cardiac function of hypertension with cardiac failure patients.Methods 96 cases of hypertension with cardiac failure patients were divided into observation group and control group with 48 cases in each group according to the random number table method.Patients in the control group were given conventional three medicines therapy including digoxin,spirolactone and furosemide,while patients in the observation group were given urapidil combined with conventional three medicine therapy.Comparing the difference of blood pressure index including SBP and DBP,cardiac function index including HR and LVEF as well as NT-proBNP levels of before and after 7 d therapy between both groups.The occurrence of adverse reactions(headache,palpitations,flushing,digestive disorders,etc.)were observed within one month after treatment.Results On the 7th day after treatment,the levels of blood indexes(SBP and DBP)in the two groups were all significantly decreased compared with those before therapy(all P<0.05).However,the comparison between both groups showed no statistical significance(all P>0.05).The levels of cardiac function indexes(HR and NT-proBNP)in the observation group were significantly more decreased and were more significant than those in the control group(all P<0.05).The LVEF level in the observation group was significantly more increased and was more significant than that in the control group(all P<0.05).Within one month after therapy,the comparison of adverse reaction occurrence rate between both groups showed no statistical significance(P>0.05).Conclusion Urapidil combined with conventional therapy is proved to be effective in improving the cardiac function of hypertension with cardiac failure patients.
