腮腺Warthin瘤的MSCT表现

MSCT Manifestations of Parotid Warthin Tumor

目的:探讨腮腺Warthin瘤的MSCT表现。方法:回顾性分析经术后病理证实的28例腮腺Warthin瘤的临床及MSCT表现。结果:28例患者中男性26例,女性2例,年龄43~82岁,平均59岁;单发21例,多发7例,共39个病灶,其中33个病灶(85%)位于腮腺浅叶后下极;CT平扫多呈圆形、卵圆形或分叶状软组织肿块,边界清晰,密度均匀或不均匀,22个病灶见不同程度囊变,多为裂隙状囊变,所有病灶均无钙化;增强扫描实性部分多呈动脉期明显强化,静脉期强化减低,囊变区无强化,其中9个病灶见包绕血管或血管贴边征。结论:病灶单发或多发,位于腮腺浅叶后下极,边界清晰,增强后"快进快出"是腮腺Warthin瘤的MSCT特征性表现。

Objective:To investigate MSCT manifestations of parotid Warthin tumor.Methods:Clinical and MSCT manifestations were retrospectively studied in 28 cases suffering from parotid Warthin tumor.Results:Of 28 cases of parotid Warthin tumor,26 patients were men and 2 were women,onset age of them ranging from 43 to 82,median age is 59;In these patients,39 lesions were found including 21 isolated lesions and 7 multiple lesions,Among these 39 lesions,33 lesion(85%)located in posterior and inferior quadrant of parotid;CT scanning showed that most of lesions are spherical/ovoid/lobulated soft tissue mass with well-defined border and homogeneous or inhomogeneous density,22 lesions showed various degrees of cystic degeneration most of which had a crack shape,however none of calcification was found in all of these lesions;In contrast-enhanced CT scans,density of the solid part of most of lesions was obviously enhanced in arterial phase and decreased in vein phase,by contrast areas of cystic degeneration showed no enhancement in density,interestingly nine of these lesions were found to be wrapped by blood vessels.Conclusion:Parotid Warthin tumors have several MSCT manifestations,including:occurrence in isolated or multiple,location in posterior and inferior quadrant of parotid,with well-defined border and"fast in and fast out"after enhancement.
