静脉血液标本的闭环管理

Closed-loop management of venous blood specimens

目的探讨闭环管理模式在静脉血液标本管理中的应用效果。方法将2017年5~7月心血管内科静脉血液标本6 332例次设为对照组,进行常规标本质量控制。将8~10月静脉血液标本7 165例次设为观察组,依据闭环式管理理论从静脉血液标本医嘱核对、标本采样、标本核对配送流程、标本流程追踪、不合格标本即时回报等5个方面进行实验室前质量控制。结果观察组由于实验室前因素导致的不合格血液标本发生率、护士原因及配送原因导致的标本不合格率显著低于对照组(均P<0.01)。结论采用闭环式管理理论对静脉血液标本从整个环节进行质量控制,有利于降低静脉血液不合格标本发生率,提高医疗及护理质量。

Objective To explore the effect of closed-loop management on the management of venous blood specimens.Methods A total of 6332 venous blood specimens in department of cardiology from May to July in 2017 was a control group,which was managed routinely to control the quality.Another 7165 specimens from August to October was an observation group,the quality before laboratory was controlled from 5 aspects based on closed-loop management theory,which were verification of blood orders,sampling of specimens,verifying of distribution process,tracing of specimens flow and immediate return of unqualified specimens.Results The rate of unqualified blood specimens caused by pre-laboratory factors and the failure rate of specimens caused by nurses and distribution reasons were significantly lower than those in the control group(P<0.01 for all).Conclusion Application of closed-loop management theory in quality control of venous blood specimens throughout the whole process,is conducive to reducing the incidence of unqualified venous blood specimens,and improving the quality of medical care and nursing.
