信任情况对黑龙江省老年人利用社区卫生服务现况的影响

Effect of Trust on Status Quo of Aging Population's Utilization of Community Health Services in Heilongjiang Province

目的:了解黑龙江省老年人对社区卫生服务的利用状况及信任情况,探讨信任情况对社区卫生服务利用的影响,为制定社区卫生服务中心发展策略提供依据。方法:采用多阶段抽样的方法,在黑龙江省抽取1003名社区老年居民进行问卷调查。结果:黑龙江省老年居民对社区卫生服务的利用率为60.1%,利用率最高的是医疗(36.7%)和免费体检(35.2%)。Spearman秩相关分析显示,对社区卫生服务机构信任的老年人更愿意利用社区卫生服务;多因素分析显示,慢性病患病情况、居住状况、是否信任社区卫生服务中心对社区卫生服务利用均存在一定程度的影响。结论:加强老年人对社区卫生服务中心的信任对提高社区卫生服务中心的利用率、促进分级诊疗有着重要作用。

Objective:To understand the status quo of utilization and trust of aging residents on community health services in Heilongjiang Province,and to discuss the impact of trust on community health service utilization,so as to provide evidence for health policy making.Methods:A questionnaire was conducted in 1003 aging residents selected by multistage sampling.The survey included the basic situation,the trust and utilization of community health services.The questionnaire was analyzed by descriptive statistics,and the influencing factors were analyzed.Results:The overall utilization rate of community health services among Heilongjiang aged people were 60.1%,the highest utilization rate of community health services were medical treatment(36.7%)and free health care checkup(35.2%).Spearman rank correlation analysis showed that older people who trust in community health services were more willing to use community health services.Multivariate analysis showed that the prevalence of chronic diseases,living conditions and the trust of community health service centers influenced to some extent the utilization of community health services.Conclusion:The improvement of elderly trust of community health service organization played an important role in improving its utilization and promoting hierarchical medical system.
