概述国内外ICU终末期患者治疗抉择的历史发展过程及法律案例。分析ICU终末期患者的治疗现状,提出目前在进行治疗抉择时针对患者及家属方面、医疗保障方面存在的问题。对目前存在的问题进行伦理分析,提出在伦理学上要遵从尊重自主、有利、公正原则。探讨终末期患者治疗抉择可能的解决措施,包括转变患者及家属尊重生命及死亡观念,临床工作中规范放弃治疗,医院成立伦理指导委员会,完善医疗保障体系等方法,从而对ICU终末期患者治疗抉择进行伦理指导与实践。

This paper summarizes the historical development and legal cases of treatment options for patients at the end of ICU.The authors analyzes the current situation of treatment of patients at the end of ICU,and puts forward some problems in the treatment of patients and their families.To carry on the ethical analysis to the existing problems,put forward the principle of respecting independence,advantage and justice in ethics.To explore possible solutions to the treatment options for terminally ill patients,including changing patients'and family members'respect for life and death,regulating the abandonment of treatment in clinical practice,setting up ethics guidance committees in hospitals,and improving medical care systems,so as to provide ethical guidance and practice for the treatment options for terminally ill patients in ICU.
