儿童膝关节色素沉着绒毛结节性滑膜炎影像学表现

Imaging manifestations of knee joint pigmented villonodular synovitis in children

目的探讨儿童膝关节色素沉着绒毛结节性滑膜炎(PVNS)的影像学表现。方法回顾性分析8例经术后病理检查证实的PVNS患儿的影像学表现,8例均行膝关节MR检查,其中增强扫描3例;膝关节X线片检查7例,膝关节超声4例。结果 X线平片显示关节囊肿胀8例,关节面硬化囊变1例,关节间隙内侧变窄1例,髌下脂肪垫密度增高1例。超声显示合并关节积液4例,髌上囊扩张明显3例,髌下囊扩张明显1例。关节滑膜不规则增厚。MRI显示病灶内T1WI呈弥漫性等或低信号、T2WI呈低信号,Medic序列呈低信号。结论 MRI具有良好的软组织分辨率,在儿童色素沉着绒毛结节性滑膜炎的诊断方面有特异性。

Objective To investigate the imaging manifestations of knee joint pigmented villlonodular synovitits(PVNS)in children.Methods The imaging manifestations of 8 children patients with PVNS who were confirmed by postoperative pathologic examination were retrospectively analyzed.The 8 patients underwent MRI examination of kuee joint,in whom,3 patients received enhancement scanning examination,7 patients received knee joint X-ray examination and 4 patients underwent knee joint ultrasonography.Results The X-ray examination results showed that there were 8 cases of joint capsule swelling(100%),1 case of articular facet hardening and vesicle changes,1 case of joint space medial narrowing and 1 case of patellar fat pad density increase.The ultrasonography demonstrated that there were 4 cases of combined joint effusion,3 cases of suprapatellar bursa extension,1 case of subpatellar bursa extension.MRI results showed that T1 weight image showed diffuse low signal intensity,T2 weight image and medic sequence showed low signal intensity.Conclusion The MRI has higher resolution for soft tissues,which has specificity in diagnosis of knee joint pigmented villlonodular synovitits in children.
