纤维支气管镜治疗ICU严重肺部感染的临床疗效探讨

Clinical Efficacy of Fiberoptic Bronchoscopy in the Treatment of Severe Pulmonary Infection in ICU

目的观察并分析纤维支气管镜治疗ICU严重肺部感染的临床疗效。方法该研究中将2017年1—6月期间该院ICU科室收治合并严重肺部感染患者共60例作为观察对象,根据数字随机表法分组,对照组、观察组分别纳入30例。纳入对照组中患者予以常规治疗,纳入观察组中患者联合予以纤维支气管镜治疗。就两组患者在临床疗效以及治疗前后血清炎性指标方面的差异进行评估与对比。结果两组间对比临床疗效,观察组总有效率93.33%(28/30)较对照组73.33%(22/30)明显更高,差异有统计学意义(P<0.05)。两组间对比血清炎性指标,观察组血清降钙素原检出值(21.6±1.0)ng/mL,血清超敏-C反应蛋白检出值(21.5±1.6)mg/L均与对照组对比差异无统计学意义(P>0.05),治疗后对比,观察组血清降钙素原检出值(5.8±0.9)ng/m L,血清超敏-C反应蛋白检出值(5.9±0.8)mg/L均较对照组明显更低,差异有统计学意义(P<0.05)。结论应用纤维支气管镜对ICU收治严重肺部感染患者进行治疗的临床疗效确切,可短期内改善患者炎性症状,有临床推广应用价值。

Objective This paper tries to observe and analyze the clinical efficacy of fiberoptic bronchoscopy in the treatment of severe pulmonary infection in ICU.Methods A total of 60 cases admitted with severe pulmonary infection from January to June 2017 in this hospital ICU Department were selected as the research objects,according to the random number table method,the control group and the observation group were included in 30 cases.The patients in the control group were given conventional treatment,and the patients in the observation group were treated with fiberoptic bronchoscopy.The clinical efficacy and serum inflammatory index differences before and after treatment of two groups of patients were evaluated and compared.Results The clinical curative effect of the observation group was 93.33%(28/30),significantly higher than the control group of 73.33%(22/30),with significant difference(P<0.05).The comparison between the two groups of serum inflammatory markers,serum procalcitonin detection value(21.6±1.0)ng/mL,serum high-sensitivity-C reactive protein detection value(21.5±1.6)mg/L,with no significant difference compared with the control group,not statistically significant(P>0.05),compared to that after treatment,the observation group of serum procalcitonin detection value(5.8±0.9)ng/mL,serum high-sensitivity-C reactive protein detection value(5.9±0.8)mg/L were significantly lower than the control group,and the difference was statistically significant(P<0.05).Conclusion The clinical efficacy of fiberoptic bronchoscopy in the treatment of patients with severe pulmonary infection in ICU is definite,which can improve the inflammatory symptoms in a short time,and has clinical application value.
