摘要
目的采用超声二维斑点追踪成像联合TEI指数评价EBSTEIN患者的右室功能。方法选取2016年1月—2017年6月来该院就诊的12例EBSTEIN患者为研究对象(EB组),并以20名正常人作为对照组(NOR组),所有研究对象接受二维斑点追踪成像及常规超声心动图检查,观察两组间右室游离壁基底段及中间段应变,右室TEI指数、三尖瓣收缩期位移(TAPSE)及组织多普勒三尖瓣环收缩期位移速度(S’)的差异。结果右心室游离壁基底段应变EB组(-17.25±1.71)%低于对照组(-24.95±0.55)%,中间段应变EB组(-14.00±1.42)%低于对照组(-24.85±0.39)%,差异有统计学意义(P<0.01)。TEI指数EB组(0.578 3±0.03)%高于正常对照组(0.319 0±0.01)%,差异有统计学意义(P<0.01)。TAPSE值EB组(27.42±1.94)mm高于正常对照组(21.75±0.50)mm,S’值EB组(20.83±0.97)cm/s高于正常对照组(14.90±0.41)cm/s,差异有统计学意义(P<0.01)。结论二维斑点追踪联合TEI指数能较好的评价EBSTEIN患者右心室功能。
Objective This paper tries to evaluate the right ventricular function with two dimensional speckletracking and TEI index in patients of EBSTEIN abnormality.Methods 12 patients with EBSTEIN abnormality admitted in this hospital from January 2016 to June 2017 were selected as the research objects(the EB group),20 cases of healthy people were selected as the control group(NOR group).All the research objects were undergone two dimensional speckletracking and routine echocardiography.The peak strain in the basal,middle segment of right free wall,TEI index,tricuspid annular plane systolic excursion(TAPSE)and tricuspid annular systolic velocity(s’)were observed,the difference were compared between the two groups.Results Compared with NOR group of(-24.95±0.55)%,the strain in the basaloffree wallis decreased in EB group was(-17.25±1.71)%,with significant difference(P<0.01).Compared with NOR group of(-24.85±0.39)%,the strain in the middle segment offree wall was decreased in EB group of(-14.00±1.42)%,(P<0.01).Compared with NOR group of(0.319 0±0.01)%,TEI index was increased in EB group of(0.578 3±0.03)%,(P<0.01)Compared with NOR group of(21.75±0.50)mm,TPASE was increased in EB group of(27.42±1.94)mm,(P<0.01).Compared with NOR group of(14.90±0.41)cm/s,S’was increased in EB group of(20.83±0.97)cm/s,and the difference was significant,(P<0.01).Conclusion Two dimensional speckle tracking cooperating with TEI index may accurately evaluate the right ventricle function of EBSTEIN abnormality.
