妊娠合并子宫肌瘤行剖宫产术中肌瘤切除的临床研究

目的将剖宫产术中肌瘤切除用于妊娠合并子宫肌瘤的治疗当中,对其临床效果进行研究分析。方法将2015年8月—2017年2月在该院接受治疗的126例妊娠合并子宫肌瘤患者作为该次研究的主要对象,将其分成两组,术中切除组和常规治疗组各63例患者,术中切除组采用剖宫产术中肌瘤切除的方法进行治疗,常规治疗组患者则仅采用子宫下段剖宫产术进行治疗,手术结束后对两组患者的术中出血量、住院时间、肌瘤变性情况、术后产褥情况以及手术并发症情况进行调查并记录。结果术中切除组中患者的平均术中出血量(176.81±32.15)m L、肌瘤变性患者4例、术后产褥患者3例,术后并发症患者8例;常规治疗组中患者的平均术中出血量(231.73±42.66)m L、肌瘤变性患者9例,术后产褥患者11例,术后并发症患者17例,前者明显优于后者,数据经检差异有统计学意义(P<0.05),在患者的住院时间方面,虽然术中切除组(5.23±1.22)d患者的住院时间短于常规治疗组(6.71±1.89)d,但两组数据经检验差异无统计学意义(P>0.05)。结论采用剖宫产术中肌瘤切除对妊娠合并子宫肌瘤患者进行治疗,能够有效提高治疗的安全性,减少患者的术中出血量,效果明显,值得推广。

Objective This paper tries to research and analyze the cesarean section of myomectomy for the treatment of pregnancy with uterine fibroids.Methods A total of 126 patients with uterine fibroids treated in this hospital from August 2015 to February 2017 were included as the main subjects of this study.The patients were divided into two groups:63 patients in each group with intraoperative resection and conventional treatment,intraoperative resection group using cesarean section resection of fibroids for treatment,conventional treatment group of patients only using the lower uterine cesarean section for treatment,at the end of surgery,the two groups of patients with intraoperative blood loss,hospital stay,Fibroid degeneration,postoperative mattress and surgical complications were investigated and recorded.Results The average surgical resection in patients with bleeding was(176.81±32.15)mL,4 patients with myoma degeneration,3 patients of postoperative puerperal,8 patients with postoperative complications;in the routine treatment group;the average amount of bleeding was(231.73±42.66)mL,9 cases of myoma degeneration,11 cases of patients with puerperal after operation.17 patients with postoperative complications,the former was superior to the latter,the test data was statistically significant(P<0.05),the hospitalization time of the resection group was(5.23±1.22)d,shorter than the conventional treatment group of(6.71±1.89)d,without significant difference(P>0.05).Conclusion The use of cesarean section of myoma resection of uterine fibroids in patients with pregnancy treatment can effectively improve the safety of treatment,reduce the amount of intraoperative blood loss,whose effect is obvious,which is worthy of promotion.
