原因不明复发性流产患者心理状况调查及心理干预对其细胞免疫功能和疗效的影响

Investigation of Mental Status and Curative Effect of Patients with Re-current Spontaneous Abortion and the Effect of Psychological Interven-tion on Cellular Immune Function

目的探讨原因不明复发性流产患者心理状况调查及心理干预对其细胞免疫功能和疗效的影响。方法选取该院2015年8月—2017年8月接诊的90例原因不明复发性流产患者作为观察组,选取同期在该院进行产检的正常妇女90名作为对照组,使用症状自评量表评价两组受检者的心理状况,观察组患者根据治疗方式分为细胞主动免疫组、黄体酮组、黄体酮联合心理干预组,使用酶联免疫吸附法检测所有患者治疗前后的IFN-γ、TNF-α、IL-4、IL-10水平,对比其妊娠结局。结果经症状自评量表调查结果显示,观察组患者的总分、抑郁、人际关系敏感均高于对照组(P<0.05);治疗前,观察组患者的IFN-γ为(331.21±45.20)μg/L、TNF-α(28.23±4.21)nmol/L水平均明显高于对照组的(271.21±32.21)μg/L、(17.22±3.56)nmol/L,IL-4(10.45±2.51)、IL-10(15.22±2.56)水平低于对照组的(14.24±3.21)、(22.46±3.21),经治疗,观察组患者的IFN-γ、TNF-α水平明显降低,而IL-4、IL-10水平明显升高(P<0.05)。结论复发性流产对女性患者的正常心理活动有很大影响,进而造成机体免疫功能紊乱,通过适当的心理干预对提高机体免疫功能,改善治疗效果,值得推广使用。

Objective This paper tries to investigate the psychological status of unexplained recurrent abortion patients and the influence and curative effect of psychological intervention on their cellular immune function.Methods 90 cases in this hospital from August 2015 to August 2017 with unexplained recurrent spontaneous abortion were selected as the observation group,90 cases of normal women were selected in this hospital as the control group,psychological status of two groups were evaluated by using SCL-90,the patients in the observation group were divided into cell active immunization group;progesterone group;progesterone combined with psychological intervention group according to the treatment methods,using enzyme-linked immunosorbent assay for detection of IFN-gamma,all patients before and after treatment of TNF-alpha and IL-4 and IL-10 level,the pregnancy outcome were compared.Results The SCL-90 survey showed that the patients in the observation group,whose total score,depression,interpersonal sensitivity were higher than that of control group(P<0.05);before treatment,observation of IFN-gamma was(331.21±45.20)μg/L,TNF-αwas(28.23±4.21)nmol/L,significantly higher than the control group of(271.21±32.21)μg/L,(17.22±3.56)nmol/L,IL-4 of(10.45±2.51),IL-10 of(15.22±2.56),lower than those of the control group of(14.24±3.21),(22.46±3.21),after treatment,the observation group of patients with IFN-γ,TNF-αlevels were significantly lower,while IL-4 and IL-10 levels increased significantly(P<0.05).Conclusion Recurrent spontaneous abortion on female patients with normal psychological activities has a great impact,and then cause the body immune dysfunction,through appropriate psychological intervention,the body’s immune function and the treatment effect can be improved,it is worth promotion and application.
