高血压病合并阻塞性睡眠呼吸暂停的血压昼夜节律研究

Circadian Rhythm of Blood Pressure in Hypertensive Patients with Obstructive Sleep Apnea

目的观察高血压合并阻塞性睡眠呼吸暂停患者的血压昼夜节律。方法选择2016年6月—2017年7月期间在该院接受治疗的高血压合并中重度阻塞性睡眠呼吸暂停患者42例和单一高血压患者42例作为研究对象,将高血压合并阻塞性睡眠呼吸暂停患者划入观察组,单一高血压患者划入对照组,其中观察组42例,对照组42例,比较两组患者的非杓型血压曲线例数、AHI指数和24 h平均血压水平。结果观察组患者22例中,血压非杓型患者12例,SBP(144.3±2.2)mm Hg,DBP(94.1±2.5)mm Hg,AHI指数(47.8±12.4);对照组患者22例中,非杓型血压曲线4例,SBP(138.2±2.6)mm Hg,DBP(89.5±2.4)mm Hg,AHI指数(1.3±1.1);组间差异有统计学意义(P<0.05)。结论高血压合并阻塞性睡眠呼吸暂停患者血压昼夜节律受到破坏,非杓型血压曲线增多,增加了患者心血管事件风险。

Objective This paper tries to observe the circadian rhythm of blood pressure in patients with hypertension complicated with obstructive sleep apnea.Methods 42 patients with moderate to severe obstructive sleep apnea and 42 patients with single hypertensive enrolled in this hospital from June 2016 to July 2017 were selected as the research objects.The patients with high blood pressure and obstructive sleep were enrolled in the observation group.Patients with single hypertensive were included in the control group,with 42 cases in each group.The number of non-dipper blood pressure curve,AHI index and 24 h blood pressure were compared between the two groups.Results Among the 22 patients in the observation group,there were 12 patients with blood pressure non-arytenoid,SBP of(144.3±2.2)mmHg,DBP of(94.1±2.5)mmHg,AHI index of(47.8±12.4);22 cases of non-dipper blood pressure curve in the control group,SBP of(138.2±2.6)mmHg,DBP of(89.5±2.4)mmHg,AHI index of(1.3±1.1);the difference was statistically significant(P<0.05).Conclusion Circadian rhythm of blood pressure in patients with hypertension and obstructive sleep apnea is damaged,and the non-dipper blood pressure curve increases,which increases the risk of cardiovascular events.
