血管超声诊断椎动脉开口处狭窄的价值分析

Value of Vascular Ultrasound in Diagnosis of Stenosis of Vertebral Artery Opening

目的分析血管超声诊断椎动脉开口处狭窄的价值。方法选择2016年7月—2017年7月来该院就诊的椎动脉开口处狭窄患者,共56例,将其设为研究组,同时选择同时期来该院进行体检的健康志愿者56名作为对照组,对患者和健康体检者的血管超声诊断进行分析比较。结果研究组的舒张末期血流速度、收缩期峰值血流速度、椎动脉内径、搏动指数和阻力指数分别为(26.76±4.32)cm/s、(80.94±13.46)cm/s、(1.43±0.26)mm、(2.57±1.13)和(0.90±0.29),其中舒张末期血流速度、收缩期峰值血流速度、搏动指数和阻力指数均比对照组更高,椎动脉内径比对照组更低,与对照组相比的差异有统计学意义(P<0.05);椎动脉开口处狭窄患者中,椎动脉开口处狭窄<50%、50%~69%和70%~99%的比例分别占66.07%、21.43%和12.50%,其中椎动脉开口处狭窄<50%所占的比例最高。结论椎动脉开口处狭窄患者和正常者的血管超声诊断结果具有较大的差异,采用血管超声诊断椎动脉开口处狭窄还能够诊断出椎动脉开口处狭窄的程度,具有重要的诊断价值。

Objective This paper tries to analyze the value of intravascular ultrasound in diagnosing stenosis of vertebral artery opening.Methods A total of 56 cases from July 2016 to July 2017 in this hospital for treatment of vertebral artery stenosis were set as the study group,and at the same time,56 cases of healthy volunteers were selected as the control group,comparative analysis of vascular ultrasound diagnosis of patients and healthy subjects was carried out.Results The end diastolic velocity,peak systolic velocity,vertebral artery diameter,pulsatility index and resistance index in the study group were(26.76±4.32)cm/s,(80.94±13.46)cm/s,(1.43±0.26)mm,(2.57±1.13)and(0.90±0.29),the end diastolic velocity,systolic the peak systolic velocity,pulsatility index and resistance index were higher than the control group,vertebral artery diameter was lower than the control group,with statistical significance(P<0.05);ostium of vertebral artery stenosis in patients with vertebral artery stenosis at the opening of<50%,50%~69%and 70%~99%respectively,accounted for 66.07%21.43%and 12.50%,the ostium of vertebral artery stenosis<50%covered the highest proportion.Conclusion Vascular ultrasound diagnosis of vertebral artery ostial stenosis patients and normal persons have bigger difference,the vascular ultrasound diagnosis of vertebral artery ostial stenosis can also diagnose ostium of vertebral artery stenosis degree,which has important diagnostic value.
