三维适形放疗联合替吉奥治疗局部晚期胰腺癌的临床分析

Clinical Analysis of Three Dimensional Conformal Radiation Therapy and S-1 in Treatment of Local advanced Pancreatic Cancer

目的观察三维适形放疗联合替吉奥治疗局部晚期胰腺癌的疗效及毒副反应。方法 38例患者随机分为联合组(19例)及单纯放疗组(19例)。单纯放疗组给予三维适形放疗,6MV X线,3~5个等中心野,剂量为50~60 Gy/25~30次,1.8~2 Gy/次,联合组的放疗射野及剂量同放疗组,同时自放疗开始起口服维康达[(80 mg/(m^2·d),2次/d,1~14,休7 d,21 d为1个周期],至少用药2个周期。分析并观察患者的肿瘤控制情况、1年生存率及不良反应。结果放化疗组有效率高于单放组(73.7%vs 42.1%),放化疗组1年生存率高于单放组(52.6%vs 21.1%),放化疗组不良反应稍高于单放组,但未出现严重的不良反应,患者可以耐受。结论三维适形放疗联合S-1是局部晚期胰腺癌较为理想的方法,疗效优于单纯放疗组。

Objective To observe the curative effect and toxic and side effects of three dimensional conformal radiation therapy and S-1 in treatment of local advanced pancreatic cancer.Methods 38 cases of patients were randomly divided into two groups with 19 cases in each,the simple chemotherapy group adopted the three dimensional conformal radiation therapy 6MV X ray,3~5 iso-center field,dose was 50~60 Gy/25~30,1.8~2 Gy/once,and the radiotherapy field and dose were the same as those in the combined group,since the beginning of radiotherapy,they took wecando orally[80 mg/(m2·d),twice a day,1~14 d,7 d off,21 days a cycle],dose for 2 weeks at least,and the tumor control situation,1-year survival rate and adverse reactions of patients were analyzed and observed.Results The effective rate in the radiotherapy group was higher than that in the single chemotherapy group(73.7%vs 42.1%),and the 1-year survival rate in the radiotherapy group was higher than that in the single radiology group(52.6%vs 21.1%),and the adverse reactions in the radiotherapy group were slightly higher than those in the single radiology group,and there were no serious adverse reactions,and the patients could be toleranced.Conclusion Three dimensional conformal radiation therapy and S-1 in treatment of local advanced three dimensional conformal radiation therapy and S-1 in treatment of local advanced pancreatic cancer is an ideal method,and the curative effect is better than that in the single radiotherapy group.
