PFNA内固定治疗股骨粗隆间骨折价值探讨

Study on Value of PFNA Internal Fixation in Treatment of Intertrochanter Fracture of Femur

目的观察PFNA内固定治疗股骨粗隆间骨折价值。方法该院骨科2011年3月—2016年12月收治64例股骨粗隆间骨折患者,根据内固定治疗方法不同将所有患者均分为实验组与对照组,对照组32例患者给予股骨近端重建钉内固定(PFN)治疗,实验组32例患者给予股骨近段抗旋髓內钉(PFNA)治疗,比较两组患者手术时间、术中出血量、术后骨折愈合时间、并发症发生情况以及患者治疗前后髋关节运动功能(Harris)评分变化情况。结果实验组患者手术时间为(40.2±8.4)min、术中出血量为(281.5±18.2)m L、术后骨折愈合时间为(54.3±3.2)d均明显优于对照组,实验组并发症发生率明显低于对照组(9.4%vs 31.3%),实验组患者髋关节运动功能(Harris)评分提升幅度明显多于对照组[(44.0±5.2)分vs(26.8±3.5)分],两组数据差异有统计学意义(P<0.05)。结论对于股骨粗隆间骨折患者给予股骨近段抗旋髓內钉(PFNA)治疗可有效缩短患者手术时间、骨折愈合时间、并发症发生少,患者术后髋关节功能恢复快。

Objective To observe the value of PFNA internal fixation in treatment of intertrochanter fracture of femur.Methods 64 cases of patients with intertrochanter fracture of femur admitted and treated in our hospital from March 2011 to December 2016 were selected and divided into two groups according to the internal fixation treatment method with 32 cases in each,the control group were treated with PFN,while the experimental group were treated with PFNA,and the operation time,intraoperative bleeding amount,postoperative fracture healing time and occurrence of complications and Harris score changes were compared between the two groups.Results The operation time,intraoperative bleeding amount,postoperative fracture time in the experimental group were obviously better than those in the control group,which were respectively(40.2±8.4)min,(281.5±18.2)mL and(54.3±3.2)d in the experimental group,and the incidence rate of complications in the experimental group was obviously lower than that in the control group(9.4%vs 31.3%),and the improvement range of Harris score in the experimental group was obviously more than that in the control group[(44.0±5.2)points vs(26.8±3.5)points],and the differences between the two groups were statistically significant(P<0.05).Conclusion For patients with intertrochanter fracture of femur,the PFNA can effectively shorten the operation time and fracture healing time with few complications,and the recovery of postoperative hip joint function is rapid.
