关节镜下微骨折术治疗膝关节软骨缺损患者的疗效及预后

Curative effect and prognosis of micro fracture surgery under arthroscopic in patients with knee joint cartilage defects

目的研究关节镜下微骨折术治疗膝关节软骨缺损患者的疗效及预后。方法将2014年3月至2015年5月我院收治的92例膝关节骨缺损患者以数字随机法分为对照组和观察组,每组46例。对照组予关节镜下行关节清理术,观察组予关节镜下微骨折术治疗,比较两组患者术后疼痛、肿胀、并发症发生情况,手术前、后HSS和Lysholm评分情况。结果观察组治疗的总有效率为93.48%(43/46),明显高于对照组的78.26%(36/46),差异具有统计学意义(P<0.05);观察组术后疼痛、肿胀发生率低于对照组(P<0.05);观察组术后并发症发生率显著低于对照组,差异具有统计学意义(P<0.05);治疗后,两组患者HSS、Lysholm评分均升高,观察组显著高于对照组,差异具有统计学意义(P<0.05)。结论关节镜下行微骨折术治疗膝关节软骨缺损的疗效显著,预后明显好于单纯关节清理术,且不增加并发症发生率。

Objective To study the effect of microfracture surgery under arthroscopy on the curative effect and prognosis of knee joint cartilage defects.Methods A total of 92 patients with knee joint cartilage defects from March 2014 to May 2015 were divided into control group and observation group according to the digital randomization,with 46 cases in each group.The control group was treated with arthrodesis,and the observation group was treated with arthroscopic micro fracture surgery.The postoperative pain,swelling,the occurrence of complication,HSS and lysholm scores before and after operation of the two group were compared.Results The total effective rate was 92.46%(43/46)in the observation group,which was significantly higher than 78.26%(36/46)in the control group,the difference was statistically significant(P<0.05).The rate of pain and swelling in the observation group was lower than those of the control group,and the incidence of postoperative complications in the observation group was significantly lower than that of the control group,the differences were statistically significant(P<0.05).After treatment,the HSS and Lysholm scores of the two groups increased,and those in the observation group was significantly higher than the control group,the differences were statistically significant(P<0.05).Conclusion The micro fracture surgery under arthroscopic in the treatment of knee joint cartilage defect has significant effect,and the prognosis is better than that of the arthrodesis,which does not increase the incidence of complications.
