黄体酮与地塞米松对胎膜早破相关因子的影响

Effects of progesterone and dexamethasone on the related factors of premature rupture of membranes

目的探讨黄体酮对胎膜早破孕妇白细胞计数(WBC)、C反应蛋白(CRP)及外周血促肾上腺皮质激素释放激素(CRH)水平的影响。方法随机将我院收治的50例胎膜早破患者分为地塞米松组(DEX组)和地塞米松加黄体酮组(DEX+P组),各25例。DEX组在常规治疗基础上加用地塞米松治疗,DEX+P组在常规治疗基础上给予地塞米松、黄体酮治疗。比较两组患者的临床效果。结果两组患者的入院时、入院24、48 h的WBC、CRP水平波动不大,组内比较差异无统计学意义(P>0.05)。两组患者的入院时、入院24、48 h的WBC、CRP水平组间比较,差异无统计学意义(P>0.05)。入院24、48 h,DEX组患者的CRH水平均明显高于入院时及DEX+P组(P<0.05)。入院24、48 h,DEX+P组患者的CRH水平与入院时比较,差异无统计学意义(P>0.05)。结论胎膜早破的患者给予地塞米松可以使外周血CRH水平得到有效提升,能够缩短分娩时间,而采用黄体酮会抑制此过程。

Objective To investigate the effect of progesterone on white blood cell(WBC),C-creative protein(CRP)and corticotropin releasing hormone(CRH)levels in pregnant women with premature rupture of membranes.Methods Fifty patients with premature rupture of membranes admitted in our hospital were randomly divided into dexamethasone group(DEX group)and dexamethasone+progesterone group(DEX+P group),with 25 cases in each group.The DEX group was given dexamethasone,and the DEX+P group was treated with dexamethasone+progesterone.The clinical effects of the two groups were compared.Results Compared with at admission,there were no significant differences in the levels of WBC and CRP of the two groups at 24 and 48 h after admission(P>0.05).At admission,24 and 48 h after admission,there were no significant differences in the levels of WBC and CRP between the two groups(P>0.05).At 24 and 48 h after admission,the CRH levels of the DEX group were higher than those at admission and the DEX+P group(P<0.05).Compared with at admission,there were no significant differences in the CRH levels of the DEX+P group at 24 and 48 h after admission(P>0.05).Conclusion Dexamethasone in patients with premature rupture of membranes significantly increased CRH levels in peripheral blood and shortened delivery time,which was inhibited by progesterone.
