青年乳腺癌与老年乳腺癌患者分子分型及临床病理特征的对比分析

Comparison of Molecular Phenotypes and Clinicopathologic Characteristics of Breast Cancer between Young and Elderly Patients

目的比较青年乳腺癌与老年乳腺癌患者的分子分型及临床病理特征。方法收集120例女性乳腺癌患者,选取其中年龄≤35岁的患者55例纳入青年组,将年龄≥60岁的同期乳腺癌患者65例纳入老年组。回顾性分析2组患者的病理类型、病理分期及分子分型的特点。结果青年组与老年组乳腺癌患者分子分型总体分布不同,差异有统计学意义(P<0.01);且青年组乳腺癌患者以三阴性型居多(36.36%),老年组乳腺癌患者以Luminal A型居多(36.92%)。青年组与老年组乳腺癌患者在肿瘤直径、临床分期、病理分级、淋巴转移方面分布不同,与老年组相比,青年组肿瘤直径>2cm的所占比例高,临床分期晚,病理分级高,容易有淋巴结转移差异均有统计学差异(P<0.05或P<0.01)。结论青年乳腺癌较老年乳腺癌更具有侵袭性,易发生淋巴结转移。青年乳腺癌较老年乳腺癌患者在病理类型、病理分期及分子分型存在明显的差异。

Objective To compare the molecular phenotypes and clinicopathologic characteristics of breast cancer between young and elderly patients.Methods A total of 120 female patients with breast cancer were divided into young group(55 patients aged≤35 years)and elderly group(65 patients aged≥60 years).Pathological types,clinical stages and molecular phenotypes were analyzed retrospectively.Results The overall distribution of molecular phenotypes of breast cancer was significantly different between young and elderly groups(P<0.01).The triple-negative breast cancer was mostly found in young group(36.36%),and luminal A breast cancer in elderly group(36.92%).Furthermore,elderly patients had higher proportion of tumor size>2 cm,more advanced clinical stage and pathological grade,and greater risk for lymph node metastasis than young patients(P<0.05 or P<0.01).Conclusion Breast cancer in young patients is more invasive and easier to metastasize to lymph nodes than that in elderly patients.There are significant differences in pathological types,clinical stages and molecular phenotypes between young and elderly breast cancer patients.
