慢阻肺患者HDAC2、IL-8、TNF-α水平及其临床意义分析

Analysis of HDAC2,IL-8 and TNF-alpha levels in patients with COPD and its clinical significance

目的观察慢阻肺患者HDAC2、IL-8、TNF-α水平变化,并分析其临床意义。方法选取2015年8月-2016年12月在我院接受治疗的慢阻肺患者为研究对象,同时选取同期在我院接受体检的健康成年人作为对照。观察两组研究对象外周单核细胞HDAC2mRNA表达情况,比较两组研究对象IL-8、TNF-α等炎症因子、血气和肺功能指标的差异,分析慢阻肺患者外周单核细胞HDAC2mRNA表达与IL-8、TNF-α水平、血气、肺功能指标的相关性。结果慢阻肺患者的外周单核细胞HDAC2mRNA相对表达量低于对照组(t=4.863,P<0.001),IL-8、TNF-α水平高于对照组(t=-225.888、-67.620,P<0.001);慢阻肺患者FVC、FEV1/FVC、MMEF和PEF水平低于对照组(t=12.106、29.139、35.426、33.291,P<0.001);慢阻肺患者pH和PaO_2低于对照组(t=2.971、9.120,P<0.05),PaCO_2高于对照组(t=-27.752,P<0.001);慢阻肺患者外周单核细胞HDAC2mRNA相对表达量与IL-8、TNF-α、PaCO_2负相关,与pH、PaO_2、FVC、FEV1/FVC、MMEF和PEF正相关(P<0.05)。结论慢阻肺患者外周单核细胞HDAC2 mRNA相对表达量较低,且与外周血炎症因子、肺功能指标和血气指标密切相关,可作为临床监测的重要指标。

Objective To observe the changes of HDAC2,IL-8 and TNF-αlevel in patients with chronic obstructive pulmonary disease(COPD)and to analyze its clinical significance.Methods COPD patients treated in our hospital from August 2015 to December 2016 were selected as the subjects,and the healthy adults who underwent physical examination in our hospital were selected as the control group.The expressions of HDAC2 mRNA in peripheral blood mononuclear cells of two groups were observed.The difference between the two groups of inflammatory factors like IL-8,TNF-α,blood gas and lung function index were compared,and the correlation of COPD patients between peripheral mRNA monocyte HDAC2 expression and IL-8,TNF-αlevels,blood gas and lung function index were analyzed.Results The relative expression of HDAC2 mRNA in peripheral monocytes of COPD patients was lower than that in the control group(t=4.863,P<0.001),and the level of IL-8 and TNF-αwere higher than those in the control group(t=-225.888,-67.620,P<0.001).The level of FVC,FEV1/FVC,MMEF and PEF in patients with COPD were lower than those in the control group(t=12.106,29.139,35.426,33.291,P<0.001).In COPD patients,PH and PaO2 were lower than those in the control group(t=2.971,9.120,P<0.05),and PaCO2 was higher than that in the control group(t=-27.752,P<0.001).The relative expression of HDAC2 mRNA in peripheral monocytes of patients with COPD was negatively correlated with IL-8,TNF-αand PaCO2,and positively correlated with PH,PaO2,FVC,FEV1/FVC,MMEF and PEF(P<0.05).Conclusion The patients with chronic obstructive pulmonary disease peripheral mononuclear cells HDAC2 mRNA relative expression is low,which is closely related with the peripheral inflammatory factors,lung function and blood gas index,can be used as an important index for clinical monitoring.
