摘要
目的探讨血管紧张素Ⅱ(AngⅡ)、心房利钠肽(ANP)、抗利尿激素(ADH)在颅脑损伤(CI)合并低钠血症患者中的检测意义。方法选取2015-12—2017-01在我院接受治疗的206例CI患者为研究对象,根据血Na+水平不同分成观察组120例(低钠血症,血Na+水平<135mmol/L)和对照组86例(血Na+水平正常)。比较2组基线资料、不同时期血清AngⅡ、ANP、ADH水平及受伤7d后的血尿素氮(BUN)、血肌酐(Cr)及血浆钠离子(Na+)水平。分析CI伴低钠血症者体内血清AngⅡ、ANP、ADH水平与Na+的关系。结果 2组基线资料比较差异无统计学意义(P>0.05)。受伤后1d、3d、7d观察组血清AngⅡ、ANP、ADH水平明显高于对照组(P<0.05)。2组血清AngⅡ、ANP水平随着受伤时间的延长而明显上升,ADH水平则明显降低(P均<0.05)。观察组血Na+水平显著低于对照组(P<0.01),2组受伤7d后的BUN及Cr水平差异无统计学意义(P>0.05)。观察组血清AngⅡ(r=-0.733,P<0.01)、ANP(r=-0.661,P<0.01)、ADH(r=-0.684,P<0.01)水平与血Na+水平均呈显著负相关性(P均<0.01)。结论血清AngⅡ、ANP、ADH水平的检测有助于临床上诊治CI伴低钠血症患者,值得推广。
Objective Our retrospective study was aimed to analyze the detective value of the serum AngⅡ,ANP and ADH in the patients with craniocerebral injury(CI)and hyponatremia.Methods Clinical data of 206 patients with CI in our hospital from December 2015 to January 2017 was retrospectively analyzed.Patients were divided into two groups according to the serum level of Na+:observe group(hyponatremia,the level of Na+<135mmol/l,120 cases)and control group(normal level of Na+,86 cases).The general information,the levels of the serum AngⅡ,ANP and ADH at 1d,3d,7d after injury and the levels of BUN,Cr,Na+at 7d after injury between two groups was analyzed.Results The general information in two groups had no statistical difference(P>0.05).The levels of the serum AngⅡ,ANP and ADH at 1d,3d,7d after injury in observe group were obviously higher than those in control group(P<0.05).The levels of the serum of AngⅡand ANP in two groups were obviously increasing as the extension of the injury time,and the level of ADH was decreasing(P<0.05).The level of Na+in observe group was obviously lower than that in control group,and the levels of BUN and Cr in two groups had no statistical difference(P>0.05).The levels of the serum of AngⅡ(r=-0.733,P<0.01),ANP(r=-0.661,P<0.01)and ADH(r=-0.684,P<0.01)in observe group had negative correlation with the levels of Na+(P均<0.01).Conclusion The detection of the serum AngⅡ,ANP and ADH is helpful for diagnosing and treating the patients with CI with hyponatremia,which is worth promoting.
