脑梗死和后循环短暂性脑缺血发作患者颈总动脉内中膜厚度和血流动力学比较

Analysis of carotid intima-media thickness and hemodynamics in patients with cerebral infarction and posterior circulation transient ischemic attack

目的探讨脑梗死和后循环短暂性脑缺血发作(TIA)患者颈动脉内中膜厚度和血流动力学的差异,明确颈动脉彩超在不同缺血性脑血管病的临床价值。方法对诊断明确的11例脑梗死组和17例后循环TIA组行颈动脉彩超检查,比较2组颈动脉内中膜厚度、收缩期血流峰值和椎动脉内径情况。结果脑梗死组颈总动脉内中膜厚度高于后循环TIA组,双侧颈内、外动脉及椎动脉收缩期峰值流速均高于后循环TIA组;所有患者左侧椎动脉内径大于右侧,但差异无统计学意义(P>0.05)。结论血管彩超在脑梗死和后循环TIA评估中有重要价值,能体现动脉硬化程度和血流动力学差异。

Objective To compare carotid intima media thickness and hemodynamics between patients with cerebral infarction and posterior circulation transient ischemic attack(TIA).Methods We studied 11 patients with cerebral infarction and 17 patients with posterior circulation TIA.The carotid intima media thickness,systolic blood flow peak and internal diameter of vertebral artery were measured with ultrasonography in all participants.Results The mean carotid intima media thickenss was significantly higher in patients with cerebral infarction as compared to patients with posterior circulation TIA.The peak flow rate of bilateral cervical internal,external artery and vertebral artery systole in patients with cerebral infarction were significantly higher than that in patients with posterior circulation TIA.The internal diameter of the left vertebral artery of all patients was greater than the right,but the difference was not statistically significant(P>0.05).Conclusion Increased carotid intima media thickenss and peak flow rate are most likely associated with cerebral infarction.Vascular color ultrasound has important value in the evaluation of cerebral infarction and posterior circulation TIA.
